CT引导下穿刺针在肺磨玻璃结节定位中的临床应用

摘要 目的研究CT引导下穿刺针在肺磨玻璃结节(ground glass nodules,GGN)定位中的临床应用。方法回顾性分析2021年11月—2022年6月在蚌埠医学院第一附属医院介入科接受GGN定位的患者103例。收集患者手术前后临床、影像学资料,统计患者定位时间及成功率、并发症发生率和胸腔镜术后相应指标。结果103例患者定位成功率99.09%(109/110);定位手术时间10.37(8.41,12.92)min;定位术后气胸率19.09%(21/110),肺内出血29.09%(32/110),胸膜反应率0.91%(1/110),疼痛率13.64%(15/110)。胸腔镜术所需时间61.00(45.25,86.00)min,术后胸腔引流管拔除时间3.00(3.00,5.00)d,术后住院时间4.00(3.00,5.00)d,术后未发生相关并发症。结论CT引导下穿刺针在GGN定位中具有操作简便、时间短、成功率高和手术准确率高等优点,但存在相关并发症。 Objective To study the clinical application of CT-guided puncture needle in the localization of pulmonary ground glass nodules(GGN).Methods A retrospective analysis of 103 patients who underwent pulmonary GGN localization in the department of interventional radiology,the First Affiliated Hospital of Bengbu Medical College from November 2021 to June 2022.The clinical and imaging data of patients before and after surgery were collected,and the localization time,success rate,complication rate and corresponding indicators after thoracoscopic surgery were counted.The application value of puncture needle in localization of pulmonary GGNs,and the correlation between pathology and imaging features of pulmonary GGNs were analyzed.Results Among those 103 patients,the rate of localization successfully was 99.09%(109/110);The localization time of pulmonary GGN was 10.37(8.41,12.92)min.The incidence of pneumothorax after localization was 19.09%(21/110),the incidence of intrapulmonary hemorrhage was 29.09%(32/110),the incidence of pleural reaction was 0.91%(1/110),the incidence of pain was 13.64%(15/110).The time required for thoracoscopy was 61.00(45.25,86.00)min,the postoperative thoracic drainage tube was pulled out for 3.00(3.00,5.00)d,and the postoperative hospital stay was 4.00(3.00,5.00)d.No postoperative complications occurred.Conclusion CTguided puncture needle in the localization of pulmonary GGN,has the advantages of simple operation,short time,high success rate,and improved surgical accuracy,but there are related complications.
