Sinking Medical Resources Helped Implement Hierarchical Diagnosis and Treatment:the Sanming Model in China

目的探讨三明市第二医院医疗资源下沉基层分院的作用,为进一步推动分级诊疗制度落实提供依据。方法通过对2018年1月—2021年12月基层分院门急诊量、县域内基层就诊率、转诊人次数和年收入等数据进行对比分析,评价总医院医疗资源下沉基层分院对推动分级诊疗落实落地的作用。结果与2018年相比,2021年基层分院门急诊量增长28%,县域内基层就诊率提高5.2%,总医院转入分院患者人数增加517%,基层分院年收入增长31%。结论总医院医疗资源下沉基层分院,能有效解决基层老百姓“看病难、看病贵、看病远、看名医难”的问题,助力分级诊疗落实落地。

Objective To explore the role of grassroots branch hospitals in assisting the implementation of hierarchical diagnosis and treatment in the sinking of medical resources of general hospital,and to provide basis for further promoting the implementation of hierarchical diagnosis and treatment system.Methods To actively explore the establishment of the basic branch mechanism of medical resources sinking in general hospitals,and to evaluate the effect of the basic branch of medical resources sinking in general hospitals on the implementation of hierarchical diagnosis and treatment through comparative analysis of the number of emergency visits,the rate of primary visits within the county,the number of upward and downward transfers,annual income and other data from January 2018 to December 2021.Results Compared with 2018,the number of emergency visits in primary hospitals increased by 28%,the rate of primary visits in county increased by 5.2%,the number of patients transferred to primary hospitals increased by 517%,and the annual income of primary hospitals increased by 31%.Conclusion The sinking of medical resources in the basic branch of general hospital can effectively solve the problems of"difficult,expensive,far away and difficult to see famous doctors"in the basic people,and help the implementation of graded diagnosis and treatment.
