腓骨高位截骨术联合关节镜下膝关节清理术在膝骨性关节炎中的应用价值

Application Value of High Fibular Osteotomy Combined with Arthroscopic Knee Debridement in Knee Osteoarthritis

目的:探讨腓骨高位截骨术联合关节镜下膝关节清理术在内翻型膝骨性关节炎中的应用价值.方法:选取2016年1月至2020年1月叶县人民医院收治的90例内翻型膝骨性关节炎患者,根据治疗方法不同分为联合组50例和对照组40例.联合组使用腓骨高位截骨术联合关节镜下膝关节清理术治疗,对照组仅使用关节镜下膝关节清理术治疗.记录两组治疗总有效率;对比治疗前、后胫骨关节指标;记录两组并发症发生率.结果:联合组治疗总有效率高于对照组(P<0.05);两组治疗前胫骨平台角、关节间隙角比较,差异无统计学意义(P>0.05),治疗后胫骨平台角明显增大,关节间隙角明显缩小,联合组胫骨平台角明显大于对照组,关节间隙角明显小于对照组(P<0.05);两组并发症发生率比较,差异无统计学意义(P>0.05).结论:腓骨高位截骨术联合关节镜下膝关节清理术治疗内翻型膝骨性关节炎效果较好,可以有效改善胫骨平台角、关节间隙角,且并发症发生率较低.

Objective:To explore the value of high fibular osteotomy combined with arthroscopic knee debridement in osteoarthritis of varus Knee-cap.Methods:A total of 90 patients with osteoarthritis of varus knee-cap admitted to Ye County People's Hospital from January 2016 to January 2020 were enrolled.According to different treatment methods,they were divided into combination group(50 cases)and control group(40 cases).The combination group was treated with high fibular osteotomy combined with arthroscopic knee debridement,while the control group was treated with arthroscopic knee debridement only.The total effective rate of treatment in the two groups were recorded.The tibial joint indexes before and after treatment were compared.The incidence of complications in the two groups was recorded.Results:The total effective rate of treatment in combination group was higher than that in control group(P<0.05).There was no significant difference in tibial plateau angle and joint clearance angle between the two groups before treatment(P>0.05).After treatment,the tibial plateau angle increased significantly,and the joint clearance angle decreased significantly.The tibial plateau angle in the combined group was significantly larger than that in the control group,and the joint clearance angle was significantly smaller than that in the control group(P<0.05).The difference in incidence of complications between the two groups was not statistically significant(P>0.05).Conclusion:The curative ffect of high fibular osteotomy combined with arthroscopic knee debridement is good on osteoarthritis of varus knee-cap.It can effectively improve the tibial plateau angle and joint clearance angle,with low incidence of complications.
