Ritgen接生手法与传统会阴保护法对经阴道分娩初产妇产程、妊娠结局的影响

Influence of Ritgen Delivery Maneuver and Traditional Perineal Protection on Stages of Labor and Pregnancy Outcomes of Primiparas during Vaginal Delivery

目的:探讨里特根(Ritgen)接生手法与传统会阴保护法对经阴道分娩初产妇产程、妊娠结局的影响.方法:选取本院产科2019年4月至2020年12月收治的166例初产妇作为研究对象,按照随机数字表法将其分为观察组87例和对照组89例.观察组产妇采用Ritgen接生手法,对照组产妇则采用传统会阴保护法助产,记录两组产妇各自的产程时间、并发症发生情况、新生儿Apgar评分及妊娠结局.结果:观察组产妇各产程时间明显短于对照组(P<0.05);观察组产妇疼痛视觉模拟评分(VAS)及产后出血量均低于对照组(P<0.05);观察组产妇会阴裂伤程度较对照组轻(P<0.05);观察组新生儿Apgar评分高于对照组,不良妊娠结局发生率低于对照组(P<0.05).结论:与传统会阴保护法相比,Ritgen接生手法更能缩短产妇各产程时间,降低产妇术后并发症发生率,减少发生不良妊娠结局的风险.

Objective:To explore the influence of Ritgen delivery maneuver and traditional perineal protection on stages of labor and pregnancy outcomes of primiparas during vaginal delivery.Methods:One hundred and sixty-six primiparas treated in Department of Obstetrics of the hospital between April 2019 and December 2020 were selected as the research subjects,and they were divided into observation group(87 cases)and control group(89 cases)according to the random number table method.The observation group received Ritgen delivery maneuver,and the control group adopted traditional perineal protection for delivery.The time of each stage of labor,occurrence of complications,neonatal Apgar score and pregnancy outcomes were recorded in the two groups and compared as well.Results:The time of each stage of labor of observation group was significantly shorter than that of control group(P<0.05).The score of visual analogue scale(VAS)and postpartum hemorrhage volume were lower in observation group than those in control group(P<0.05).The perineal laceration degree of observation group'was milder than that of control group(P<0.05).The neonatal Apgar score in observation group was higher than that in control group while the incidence rates of adverse pregnancy outcomes were lower than those in control group(P<0.05).Conclusion:Compared with traditional perineal protection,Ritgen delivery maneuver can better shorten the time of each stage of labor,reduce the incidence rates of postoperative complications and lower the risk of adverse pregnancy outcomes.
