妊娠期糖尿病合并重度子痫前期患者糖脂水平、血液流变学变化及围生结局分析

Analysis of Glycolipid Levels,Hemorheological Changes and Perinatal Outcome in Patients with Gestational Diabetes Mellitus Combined with Severe Preeclampsia

目的:研究妊娠期糖尿病(GDM)合并重度子痫前期患者糖脂水平、血液流变学变化及围生结局.方法:选取2017年5月至2020年5月在本院治疗的75例GDM合并重度子痫前期患者为观察组,同期在本院检查的69例糖尿病妊娠孕妇为糖尿病组、本院孕检的正常妊娠孕妇82例为对照组.比较三组糖脂代谢指标、血液流变学指标及围产期结局,分析影响患者不良围生结局的相关危险因素.结果:观察组空腹血糖(FBG)、餐后2小时血糖(2hPBG)、血浆黏度、全血黏度、纤维蛋白原均高于对照组(P<0.05);三组不良围生结局总发生率比较:观察组、糖尿病组、对照组依次降低(P<0.05).结论:临床可通过针对糖代谢异常而合理控制血糖,并加强监测血液流变学指标,对患者进行针对性早期积极治疗,以降低围生期不良结局发生率.

Objective:To study the glycolipid levels,blood rheology changes and perinatal outcome in patients with gestational diabetes mellitus(GDM)combined with severe preeclampsia.Methods:Seventy-five cases of CDM patients with severe preeclampsia who were treated in this hospital from May 2017 to May 2020 were selected as the observation group,69 pregnant women with diabetes who were examined in this hospital during the same period were selected as the diabetes group,and 82 pregnant women with normal pregnancy examined in our hospital were selected as a control group.Glucose and lipid metabolism indexes,hemorheology indexes and perinatal outcome among the three groups were compared;the related risk factors that affected the patients'adverse perinatal outcome were analyzed.Results:Fasting blood glucose(FBG),2hours postprandial blood glucose(2hPBG),plasma viscosity,whole blood viscosity,and fibrinogen in the observation group were higher than those in the control group(P<0.05).Comparison of the total incidence of adverse perinatal outcomes among the three groups was as fllows:the incidence of adverse perinatal comes in observation group,diabetes group,control group decreased in turn(P<0.05).Conclusion:Clinically,blood glucose can be reasonably controlled through checking abnormal glucose metabolism,and blood rheological indicators can be strengthened to monitor,early and active treatment of patients can be carried out to reduce the incidence of adverse perinatal outcomes.
