股前外侧flow-through穿支皮瓣修复四肢软组织缺损的临床应用

Clinical Application of Flow-through Anterolateral Thigh Perforator Flap in Repairing Soft Tissue Defects of Extremities

目的:探讨股前外侧flow-through穿支皮瓣修复四肢软组织缺损的临床效果.方法:自2012年7月至2020年6月对25例严重四肢软组织缺损伴深部组织外露及缺损患者采取股前外侧flow-through穿支皮瓣修复.结果:25例皮瓣全部成活;1例出现静脉危象,立即探查发现伴行静脉吻合口形成长段血栓,改为皮瓣静脉吻合大隐静脉,皮瓣成活.术后半年2例皮瓣臃肿,给予整形.25例患者随访1~2年,平均15个月,皮瓣色泽质地好,外形满意,未见明显萎缩,皮瓣感觉(2PD)恢复明显、供区功能及外观方面满意.结论:股前外侧flow-through穿支皮瓣是早期修复伴有主干血管部分缺损的肢体创面的理想方法.

Objective:To investigate the clinical effect of flow-through anterolateral thigh perforator flap in repairing soft tissue defects of extremities.Methods:From July 2012 to June 2020,25 Patients with soft tissue defects of extremities with deep tissue exposure and defects were repaired with flow-through anterolateral thigh perforator flap.Results:All 25 flaps survived;Vascular crisis occurred in 1 case.And thrombosis was found in the venous anastomosis,the flap survived after re-anastomosis of saphenous vein.Six months after operation,2 cases had swollen flaps and were given plastic surgery.Twenty-five patients were followed up for 1~2 years,with an average of 15 months.The flap had good texture,satisfactory appearance with no obvious lap.Sensation(2PD)recovered significantly,meanwhile,the function and appearance of donor site were satisfactory.Conclusion:Flow-through anterolateral thigh perforator flap is an ideal option for soft tissue defects of extremities with defect of main vessel.
