曼月乐联合高效价孕激素用于治疗保留生育功能的子宫内膜不典型增生的临床研究

Clinical study of Mirena combined with high-potency progesterone in the treatment of atypical endometrial hyperplasia with fertility preservation

目的探究曼月乐联合高效价孕激素用于治疗保留生育功能的子宫内膜不典型增生的临床效果。方法选取2016年2月至2021年10月本院确诊的子宫内膜不典型增生并要求保留生育功能的90例患者为研究对象,按照随机数字表法分为对照组和研究组各45例。对照组单独应用高效价孕激素治疗,研究组在对照组基础上联合应用曼月乐治疗。结果研究组临床治疗有效率97.78%高于对照组77.78%,差异具有统计学意义(P<0.05)。治疗后研究组月经周期、月经持续时间与子宫内膜厚度均低于对照组,差异具有统计学意义(P<0.05)。研究组的备孕成功率51.11%、活产儿率40.00%均高于对照组28.89%、20.00%,差异具有统计学意义(P<0.05)。结论具有明确保留生育功能的子宫内膜不典型增生患者联合应用曼月乐、高效价孕激素治疗,具有显著治疗效果,月经周期、月经持续时间与子宫内膜厚度改善显著,妊娠成功率及活产率显著更高,更具应用价值。

Objective To explore the clinical effect of Manyuele combined with high potency progestogen in the treatment of atypical hyperplasia of endometrium with retained fertility.Methods A total of 90 patients with atypical endometrial hyperplasia who were diagnosed in our hospital from February 2016 to October 2021 and required to preserve fertility were selected as the research objects,and divided into the control group and the study group with 45 cases in each group according to the random number table method.The control group was treated with high-potency progesterone alone,and the study group was treated with Mirena on the basis of the control group.Results The effective rate of clinical treatment in the study group was 97.78%higher than that in the control group,77.78%,and the difference was statistically significant(P<0.05).After treatment,the menstrual cycle,menstrual duration and endometrial thickness in the study group were lower than those in the control group,and the difference was statistically significant(P<0.05).The success rate of pregnancy preparation in the study group was 51.11%,and the live birth rate was 40.00%,which were higher than those in the control group,which were 28.89%and 20.00%,and the differences were statistically significant(P<0.05).Conclusion The combination of Manyuele and high-valent progestogen has a significant therapeutic effect on patients with atypical hyperplasia of endometrium,which has a definite fertility retention function.The menstrual cycle,menstrual duration and endometrial thickness are significantly improved,and the pregnancy success rate and live birth rate are significantly higher,so it has more application value.
