某院静脉用药调配中心不合理用药医嘱的帕累托图分析

Pareto Chart Analysis of Irrational Medication Orders in Pharmacy Intravenous Admixture Service of a Hospital

目的通过对我院静脉用药调配中心不合理用药医嘱的帕累托图分析,了解不合理用药情况及原因,为临床合理用药提供参考。方法收集2017-2020年我院静脉用药调配中心审核的不合理用药医嘱609条,应用帕累托图分析导致医嘱不合理的主要因素、次要因素及一般因素。结果溶媒选择(占比42.20%)、给药浓度(占比17.57%)及给药剂量不适宜(占比14.29%)是造成不合理用药医嘱的主要因素;给药途径不适宜(占比10.67%)为次要因素;配伍禁忌(占比9.03%)、用药频次(占比4.27%)及药物选择不适宜(占比1.97%)为一般因素。结论运用帕累托图对我院静脉用药调配中心不合理用药医嘱进行分析,可直接发现主要因素,便于有针对性地进行干预,提高临床用药的合理性。

Objective To investigate the number and causes of irrational medication orders through Pareto chart analysis of the irrational medication orders at the pharmacy intravenous admixture services so as to provide reference for clinical rational medications.Methods A total of 609 irrational medication orders at the pharmacy intravenous admixture services between 2017 and 2020 were collected,and the various causes of irrational medication orders were analyzed by the Pareto chart.Results Selection of wrong solvents(42.20%),inappropriate drug concentration(17.57%)and dosage of drugs(14.29%)were dominating in irrational medication orders,followed by wrong routes of administration(10.67%),incompatibility(9.03%),irrational frequency of administration(4.27%)and drug selection(1.97%).Conclusion Pareto chart analysis of irrational medication orders can directly identify the main causes,which can facilitate targeted interventions and improve the rationality of clinical use of drugs.
