金匮肾气丸在老年骨质疏松症患者中的疗效及对症候积分与骨密度的影响

Therapeutic Effect of Jinkui Shenqi Pill in Elderly Patients with Osteoporosis and Its Effect on Symptom Score and Bone Mineral Density

目的:研究探讨金匮肾气丸在老年骨质疏松症患者中的疗效及对症候积分与骨密度的影响。方法:选取2020年12月至2021年12月我院收治的90例老年骨质疏松症患者为研究对象,随机分为观察组、对照组,每组各45例。对照组以常规西药治疗,观察组在对照组治疗基础上增加金匮肾气丸治疗。综合评估比较两组患者的临床疗效、中医症候积分、骨密度、骨代谢指标、炎症损伤指标、不良反应发生率等。结果:观察组治疗总有效率为91.11%(41/45),高于对照组的73.33%(33/45)(P<0.05)。治疗后,观察组患者中医症候积分(包括腰膝酸软、下肢痿弱、胸闷气短)低于对照组,而腰椎L2~L4骨密度高于对照组(P<0.05);观察组患者骨代谢指标I型前胶原氨基末端前肽(PINP)、抗酒石酸酸性磷酸酶(TRACP)水平均低于对照组,骨形成蛋白6(BMP6)水平高于对照组(P<0.05)。治疗后观察组患者炎症因子4D同型二聚体(SM4D)、丝裂原活化蛋白激酶(MAPK)、缺氧诱导因子-1(HIF-1)水平均低于对照组(P<0.05)。治疗期间观察组不良反应发生率为13.33%(6/45),低于对照组[8.89%(4/45)](P>0.05)。结论:金匮肾气丸在老年骨质疏松症患者中的治疗效果良好,能够改善患者的主要中医症候,提高患者骨密度,改善患者骨代谢指标,抑制炎症因子表达,治疗安全性良好。

Objective:To study the therapeutic effect of jinkui shenqi pill in elderly patients with osteoporosis and its effect on symptom score and bone mineral density.Methods:90 elderly patients with osteoporosis treated in our hospital from December 2020 to December 2021 were randomly divided into the observation group and the control group,with 45 cases in each group.The control group was treated with conventional western medicine,and the observation group was treated with jinkui shenqi pill on the basis of the treatment of the control group.The clinical efficacy,traditional Chinese medicine(TCM)symptom score,bone mineral density,bone metabolism index,inflammatory injury index and the incidence of adverse reactions of the two groups of patients were comprehensively evaluated and compared.Results:The total effective rate in the observation group was 91.11%(41/45),which was higher than 73.33%(33/45)in the control group(P<0.05).After treatment,the score of TCM symptoms in the observation group was lower than that in the control group,such as sore waist and knees,flaccid lower limbs,chest tightness and shortness of breath,and the bone mineral density of lumbar L2-L4 of patients in the observation group was higher than that in the control group(P<0.05).The level of bone metabolism indexes type I procollagen amino terminal propeptide(PINP)and tartrate resistant acid phosphatase(TRACP)of patients in the observation group were lower than those in the control group,and the level of bone morphogenetic protein 6(BMP6)of patients in the observation group was higher than that in the control group(P<0.05).After treatment,the levels of inflammatory factor 4D homodimer(SM4D),mitogen activated protein kinase(MAPK)and hypoxia inducible factor-1(HIF-1)of patients in the observation group were lower than those in the control group(P<0.05).During the treatment,the incidence of adverse reactions in the observation group was 13.33%(6/45),which was lower than that of the control group[8.89%(4/45)](P>0.05).Conclusion:Jinkui shenqi pill has a good therapeutic effect in elderly patients with osteoporosis,which can improve the main TCM symptoms,the bone mineral density,and the level of bone metabolism index,inhibit the expression of inflammatory factors,and the therapeutic safety is good.
