合谷刺的“五体”临床应用

Clinical Application of Hegu Needling in “Five Body Constituents”

合谷刺源自于《灵枢·官针》的“五刺”,其操作方法简单,刺激量大,作用范围广,镇痛效果强,能够改善局部组织血运和代谢,松解经筋,调节肌肉张力。在临床实践中,采用合谷刺针法治疗的优势病种主要集中于软组织疾病及神经内科,包括中风后遗症、肩关节周围炎、颈椎病、肌筋膜炎、网球肘、面神经麻痹等。单独应用合谷刺一般较少,常与其他治疗手段并用,选用阿是穴或刺激点,配合辩证使用相关经穴在“肌痹”中使用合谷刺针法较为广泛,而在其他“五体”(皮、脉、肉、筋、骨)中研究较少,包括腕管综合征、踝关节扭伤、腰椎间盘突出、斑秃、皮神经炎、带状疱疹、惊恐等。因此本研究基于“五体”总结合谷刺相关临床应用并提出展望,旨在为进一步了解合谷刺适应症和应用方式提供参考。

Hegu needling is one of the five techniques of needling in“Lingshu·Guanzhen”.Because of its simple operation,large amount of stimulation,wide range of action and strong analgesic effect,Hegu needling can improve local tissue blood circulation and metabolism,release meridian tendons and regulate muscle tension.In clinical application,it has advantages in treating soft-tissue disorders and neurology disease,including stroke sequelae,scapulohumeral periarthritis,cervical spondylosis,myofascitis,external epicondylitis of humerus,facial nerve paralysis and so on.Generally,it is rarely used alone,and is often used in combination with other therapeutic methods.The acupoints of Hegu needling are mostly Ashi points or stimulation points.It is usually used in"muscle arthralgia"based on syndrome differentiation and cooperate with related acupuncture points,but less studied in other"five body constituents"(skin,vessel,muscle,tendons,bone),including carpal tunnel syndrome,ankle sprain,lumbar disc herniation,alopecia areata,cutaneous neuritis,herpes zoster,panic,etc.Therefore,this study summarizes the clinical application of Hegu needling from"five body constituents"and puts forward the application prospects,in order to provide reference for further understanding the indications and application mode of Hegu needling.
