达英-35、炔诺酮与黄体酮治疗围绝经期异常子宫出血的疗效及安全性分析

Efficacy and safety of Daine-35,norethisterone and progesterone in the treatment of perimenopausal abnormal uterine bleeding

目的分析并对比炔雌醇环丙孕酮片(达英-35)、炔诺酮与黄体酮治疗围绝经期异常子宫出血的临床疗效及安全性。方法选取93例围绝经期异常子宫出血患者,根据治疗方法不同分为A组、B组、C组,每组31例。其中A组应用达英-35治疗,B组应用炔诺酮治疗,C组患者应用黄体酮治疗。对比三组临床疗效、治疗前后子宫内膜厚度及性激素水平,并观察安全性。结果A组患者治疗总有效率96.77%明显高于B组90.32%、C组74.19%,差异有统计学意义(P<0.05)。治疗前,三组子宫内膜厚度比较,差异无统计学意义(P>0.05);治疗后,A组、B组患者子宫内膜厚度均显著薄于C组,差异有统计学意义(P<0.05)。治疗前,三组卵泡刺激素(FSH)、促黄体生成素(LH)及雌二醇(E_(2))水平比较,差异无统计学意义(P>0.05);治疗后,A组、B组FSH、LH及E_(2)水平均显著低于C组,差异有统计学意义(P<0.05)。三组均无严重不良反应发生。结论达英-35与炔诺酮治疗围绝经期异常子宫出血的效果均显著,尤其是达英-35的治疗效果更加显著,有利于改善子宫内膜厚度,调节性激素水平,且安全性高,值得临床推广与应用。

Objective To analyze and compare the clinical efficacy and safety of ethinyl estradiol cyproterone tablets(Daine-35),norethinone and progesterone in the treatment of perimenopausal abnormal uterine bleeding.Methods 93 patients with perimenopausal abnormal uterine bleeding were selected and divided into group A,group B and group C according to different treatment methods,with 31 cases in each group.Group A was treated with Daine-35,group B was treated with norethinone,and group C was treated with progesterone.The clinical efficacy,endometrial thickness and sex hormone levels before and after treatment were compared between the three groups,and the safety was observed.Results The total effective rate of group A(96.77%)was significantly higher than that of group B(90.32%)and group C(74.19%)(P<0.05).Before treatment,there was no significant difference in endometrial thickness among the three groups(P>0.05).After treatment,the endometrial thickness in groups A and B was significantly thinner than that in group C,and the difference was statistically significant(P<0.05).Before treatment,there was no significant difference in the levels of follicle-stimulating hormone(FSH),luteinizing hormone(LH)and estradiol(E_(2))among the three groups(P>0.05).After treatment,the levels of FSH,LH and E_(2)in groups A and B were significantly lower than those in group C,and the differences were statistically significant(P<0.05).No serious adverse reactions occurred in the three groups.Conclusion Daine-35 and norethisterone have significant effects on perimenopausal abnormal uterine bleeding,especially the treatment effect of Daine-35 is more significant,which is beneficial to improve endometrial thickness,regulate sex hormone levels,and has high safety,which is worthy of clinical promotion and application.
