雌激素局部与全身序贯治疗老年性阴道炎的价值

The value of local and systemic sequential application of estrogen in the treatment of senile vaginitis

目的 探析雌激素局部与全身序贯治疗老年性阴道炎的临床疗效。方法 选取80例老年性阴道炎患者,根据随机数字表法分为对照组与研究组,各40例。对照组采用雌激素乳膏治疗,研究组采用雌激素局部与全身序贯治疗,比较两组临床疗效。结果 治疗前,两组阴道炎症状评分、阴道脱落细胞成熟指数、阴道pH值比较,差异无统计学意义(P>0.05);治疗后,研究组阴道炎症状评分、阴道pH值低于对照组,阴道脱落细胞成熟指数高于对照组,差异有统计学意义(P<0.05)。治疗前,两组孕酮、雌二醇及卵泡刺激素水平比较,差异无统计学意义(P>0.05);治疗后,两组孕酮、雌二醇及卵泡刺激素水平均优于治疗前,研究组孕酮、卵泡刺激素水平低于对照组,雌二醇水平高于对照组,差异有统计学意义(P<0.05)。研究组不良反应发生率5.0%低于对照组的20.0%,差异有统计学意义(P<0.05)。结论 雌激素局部与全身序贯治疗老年性阴道炎患者,可减轻阴道炎症状,改善血清激素水平,且安全性较高,值得临床推广与应用。

Objective To explore the clinical effect of local and systemic estrogen sequential treatment of senile vaginitis.Methods 80 patients with senile vaginitis were selected and divided into control group and study group according to random number table method,40 cases in each group.The control group was treated with estrogen cream,and the study group was treated with local and systemic estrogen sequential therapy.The clinical efficacy of the two groups was compared.Results Before treatment,there were no significant differences in vaginitis symptom score,vaginal exfoliated cell maturation index and vaginal pH value between the two groups(P>0.05).After treatment,the vaginitis symptom score and vaginal pH value of the study group were lower than those of the control group,and the vaginal exudated cell maturation index was higher than that of the control group,and the differences were statistically significant(P<0.05).Before treatment,there was no significant difference in the levels of progesterone,estradiol and follicle stimulating hormone between the two groups(P>0.05).After treatment,the levels of progesterone,estradiol and follicle stimulating hormone in the two groups were better than those before treatment.The levels of progesterone and follicle stimulating hormone in the study group were lower than those in the control group,and the levels of estradiol were higher than those in the control group,and the differences were statistically significant(P<0.05).The incidence of adverse reactions in the study group was 5.0%lower than that in the control group(20.0%),and the difference was statistically significant(P<0.05).Conclusion Local and systemic estrogen sequential treatment of senile vaginitis can reduce the symptoms of vaginitis,improve serum hormone levels,and has high safety which is worthy of clinical promotion and application.
