异型钢板固定对足部Lisfranc损伤的疗效探究

Effect of Special-shaped Plate Fixation on Foot Lisfranc Injury

目的:探究异型钢板固定对足部Lisfranc损伤的疗效。方法:以2018年1月-2019年12月在中山市中医院接受治疗的足部Lisfranc损伤患者96例作为研究对象,按照随机数字表法分为对照组(48例)和异型钢板组(48例)。其中对照组给予微型钢板内固定治疗,异型钢板组给予异型钢板内固定治疗。比较两组疼痛情况[疼痛视觉模拟评分法(VAS)]、美国足踝外科协会(American Orthopedic Foot and Ankle Society,AOFAS)踝-后足评分、围手术期情况(失血量、透视次数及手术时间)、临床疗效和不良反应发生情况。结果:两组术后6个月VAS评分、AOFAS踝-后足评分比较差异无统计学意义(P>0.05),内固定物取出后3个月VAS评分均明显降低(P<0.05),但是组间比较差异无统计学意义(P>0.05);内固定物取出后3个月AOFAS踝-后足评分明显升高(P<0.05),且异型钢板组明显高于对照组(P<0.05);异型钢板组手术时间与对照组相比明显较短(P<0.05),两组失血量及透视次数比较差异无统计学意义(P>0.05);异型钢板组优良率(72.92%)明显低于对照组(89.58%)(P<0.05);两组不良反应发生率比较差异无统计学意义(P>0.05)。结论:异型钢板固定用于足部Lisfranc损伤患者可有效减轻疼痛,提高足踝部功能,促进术后恢复,具有一定的应用价值。

Objective:To explore the effect of special-shaped plate fixation on foot Lisfranc injury.Method:Ninety-six patients with foot Lisfranc injury who were treated in Zhongshan Hospital of Traditional Chinese Medicine were taken as the research objects.According to the random number table method,they were divided into control group(48 cases)and special-shaped plate group(48 cases).The control group was given micro steel plate internal fixation,and the special-shaped steel plate group was given special-shaped steel plate internal fixation.The pain conditions[pain visual analogue scale(VAS)],American Orthopedic Foot and Ankle Society(AOFAS)ankle-hindfoot score,perioperative situation(blood loss,number of fluoroscopy and operation time),clinical efficacy and adverse reactions were compared between the two groups.Result:There were no significant differences in VAS scores and AOFAS ankle-hindfoot scores between the two groups at 6 months after operation(P>0.05),the VAS scores were significantly reduced 3 months after the plate was removed(P<0.05),while there were no significant differences between the groups(P>0.05);the AOFAS ankle-hindfoot score was significantly increased 3 months after the plate was removed(P<0.05),and the special-shaped plate group was significantly higher than the control group(P<0.05).Compared with the control group,the operation time of the special-shaped plate group was significantly shorter(P<0.05),and there were no significant differences in blood loss and the number of fluoroscopy between the two groups(P>0.05);the excellent and good rate of the special-shaped steel plate group(72.92%)was significantly lower than that of the control group(89.58%)(P<0.05);there was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:The application of special-shaped plate fixation for patients with foot Lisfranc injury can effectively relieve pain,improve foot and ankle function,and promote postoperative recovery.It has certain application value.
