宜胃止痛抗幽汤治疗Hp感染性胃脘痛临床观察

Clinical Observation on Yiwei Zhitong Kangyou Decoction in the Treatment of Hp Infectious Gastric Pain

目的探讨自拟宜胃止痛抗幽汤加减治疗幽门螺杆菌(Hp)感染性胃脘痛的应用价值。方法选取2019年6月—2020年6月辽宁省新宾满族自治县人民医院收治的Hp感染性胃脘痛患者84例作为研究对象,分为对照组(42例)和治疗组(42例),分组方法为随机数字表法。对照组给予奥美拉唑、阿莫西林及克拉霉素治疗,治疗组在此基础上加用自拟宜胃止痛抗幽汤加减治疗。对比2组的临床疗效、Hp根除率、胃液指标水平及不良反应。结果治疗组总有效率、Hp根除率与对照组比较显著升高(P<0.05)。治疗3周后,治疗组三羟胆酸、氨基己糖水平与对照组比较,显著降低,胃蛋白酶、胃液分泌性免疫球蛋白水平与对照组比较显著升高(P<0.05)。治疗组不良反应总发生率与对照组比较差异无统计学意义(P>0.05)。结论自拟宜胃止痛抗幽汤治疗Hp感染性胃脘痛,能提升Hp根除率,调节胃液分泌,且安全可靠,值得临床推广使用。

Objective To explore the application value of Yiwei Zhitong Kangyou decoction in adjuvant treatment of patients with epigastric pain caused by Helicobacter pylori(Hp)infection.Methods A total of 84 patients with Hp infectious epigastric pain admitted to Xinbin Manchu Autonomous County People’s Hospital from June 2019 to June 2020 were selected as the research objects,and they were divided into a control group(n=42)and a treatment group(n=42).The grouping method is random number table method.The control group was treated with omeprazole,amoxicillin and clarithromycin.On this basis,the treatment group was treated with Yiwei Zhitong Kangyou decoction.The clinical effect,Hp eradication rate,gastric juice index level and adverse reactions were compared between the two groups.Results The total effective rate and Hp eradication rate of the treatment group were significantly higher than those of the control group(P<0.05).After 3 weeks of treatment,the levels of trihydroxycholic acid and hexosamine in the treatment group were significantly lower than those in the control group,while the levels of pepsin and secretory immunoglobulin in gastric juice were significantly higher than those in the control group(P<0.05).There was no significant difference in the total incidence of adverse reactions between the treatment group and the control group(P>0.05).Conclusion Yiwei Zhitong Kangyou decoction can assist in the treatment of epigastric pain caused by Hp infection,which can increase the eradication rate of Hp,regulate gastric juice secretion,and is safe and reliable.It is worthy of clinical application.
