基于个案管理模式的渐进性康复护理对全面性发育迟缓患儿的干预效果

Intervention effect of progressive rehabilitation nursing based on case management model on children with comprehensive developmental retardation

目的研究基于个案管理模式的渐进性康复护理对全面性发育迟缓患儿的干预效果。方法选取2020年7月至2021年6月在榆林市第二医院就诊的70例全面性发育迟缓患儿作为研究对象,采用随机数表法将所有患儿分为研究组和对照组各35例。对照组患儿给予常规康复护理,观察组患儿给予基于个案管理模式的渐进性康复护理,每周1次,每次30 min,来院连续训练3个月。训练3个月后比较两组患儿护理前后的护理效果;选择Gesell小儿神经心理发育量表测评患儿的发育商,华东师范大学监制的启音博士仪器评定患儿的口面部运动功能,日常生活能力量表(ADL)评定患儿的日常生活活动能力。结果研究组患儿的护理总有效率为88.57%,明显高于对照组的65.71%,差异有统计学意义(P<0.05);护理前,两组患儿的适应性、言语能力、粗大运动、精细运动与个人社交行为的发育商指标比较差异均无统计学意义(P>0.05);护理后,两组患儿的适应性、言语能力、粗大运动、精细运动与个人社交行为的发育商指标均明显提高,且研究组患儿分别为(65.24±5.43)分,(85.54±3.91)分,(82.65±4.21)分,(81.48±4.56)分,(75.23±5.87)分,明显高于对照组的(58.12±4.89)分、(76.59±3.98)分、(76.29±4.07)分、(75.46±4.49)分、(64.29±6.21)分,差异均有统计学意义(P<0.05);护理前,两组患儿的口面部运动能力与日常生活活动能力比较差异均无统计学意义(P>0.05);护理后,两组患儿的口面部运动能力与日常生活活动能力均明显提高,研究组患儿的口面部运动能力与日常生活活动能力分别为(78.26±6.12)分、(76.21±3.91)分,明显高于对照组的(70.16±5.84)分、(65.29±5.28)分,差异均有统计学意义(P<0.05)。结论基于个案管理模式的渐进性康复护理能有效改善全面性发育迟缓患儿的发育商,提高口面部运动功能和日常生活活动能力,干预效果显著,值得临床推广。

Objective To study the intervention effect of progressive rehabilitation nursing based on case management model on children with comprehensive developmental retardation.Methods Seventy children with comprehensive developmental retardation who were hospitalized in Yulin No.2 Hospital from July 2020 to June 2021 were selected as the study objects.All children were divided into the study group and the control group by random number table method,each with 35 children.The children in the control group were given routine rehabilitation nursing,while those in the observation group were given progressive rehabilitation nursing based on case management mode,once a week,30 minutes each time,for 3 months.After 3 months of training,the nursing effects of the two groups of children before and after nursing were compared.Gesell Development Scale(GDS)was used to evaluate the development quotient of the children.The Qiyin doctor instrument supervised by East China Normal University was used to evaluate the oral and facial motor function of the children,and Activity of Daily Living Scale(ADL)was used to evaluate the children's activities of daily living.Results The total effective rate of nursing in the study group was 88.57%,which was significantly higher than 65.71%in the control group(P<0.05).Before nursing,the developmental quotient indexes of adaptability,speech ability,gross motor,fine motor,and personal social behavior showed no significant difference between the two groups(P>0.05);after nursing,the developmental quotient indexes in the two groups were significantly improved,and the scores in the study group were(65.24±5.43)points,(85.54±3.91)points,(82.65±4.21)points,(81.48±4.56)points,(75.23±5.87)points,respectively,which were significantly higher than(58.12±4.89)points,(76.59±3.98)points,(76.29±4.07)points,(75.46±4.49)points,(64.29±6.21)points of the control group;the differences were statistically significant(P<0.05).Before nursing,the oral and facial motor abilities and activities of daily living showed no significant difference between the two groups(P>0.05);after nursing,the two indexes were significantly improved in the two groups,and they were(78.26±6.12)points and(76.21±3.91)points in the study group,respectively,which were significantly higher than(70.16±5.84)points and(65.29±5.28)points in the control group;the differences were statistically significant(P<0.05).Conclusion The progressive rehabilitation nursing based on case management model can effectively improve the developmental quotient of children with comprehensive developmental retardation,improve the oral and facial motor function and activities of daily living,with significant intervention effect,which is worthy of clinical promotion.
