职业性噪声性听力损失人群氧化损伤指标变化

Variation in oxidative injury markers in workers with occupational noise-induced hearing loss

摘要 目的探讨职业性噪声性听力损失(ONHL)人群体内氧化损伤指标变化,筛选ONHL人群的效应生物标志物。方法采用判断抽样方法,选择某大型机械维修企业288名男性噪声作业工人为研究对象,对其进行纯音听阈测试,根据听力损失情况并按工种、年龄、工龄1∶1配对分为听力损失组和听力正常组各144人。测定2组人群血清中总超氧化物歧化酶(T-SOD)、铜锌超氧化物歧化酶(CuZn-SOD)、锰超氧化物歧化酶(Mn-SOD)、谷胱甘肽硫转移酶(GST)和过氧化氢酶(CAT)活力。结果听力损失组人群血清GST、T-SOD、Mn-SOD和CAT活力均低于听力正常组(P值均<0.05)。多因素1∶1配对logistic回归分析结果显示,在控制吸烟和饮酒因素的影响后,ONHL与血清GST、Mn-SOD、CAT活力均存在关联(P值均<0.05);GST、Mn-SOD和CAT活力越高,罹患ONHL的风险越低。CuZn-SOD活力与ONHL无关联(P>0.05)。结论血清GST、Mn-SOD和CAT等抗氧化酶水平均与ONHL有关,上述抗氧化酶可作为ONHL人群的效应生物标志物。 Objective To explore the changes of oxidative damage indicators in workers with occupational noise-induced hearing loss(ONHL),and to screen possible biomarkers of ONHL.Methods A total of 288 male workers exposed to noise in a large machinery maintenance enterprise were selected as the research subjects using the method of judgment sampling.After pure tone hearing threshold test,they were divided into hearing loss group and normal hearing group according to the hearing loss and 1∶1 pairing design of type of work,age,length of service,with 144 people in each group.The activities of total superoxide dismutase(T-SOD),copper-zinc superoxide dismutase(CuZn-SOD),manganese superoxide dismutase(Mn-SOD),glutathione S-transferase(GST)and catalase(CAT)in serum of workers of the two groups were measured.Results The activities of GST,T-SOD,Mn-SOD and CAT in workers of the hearing loss group were lower than those in the normal hearing group(all P<0.05).The 1∶1 logistic regression analysis of paired conditions showed that there was an association between ONHL and activities of GST,Mn-SOD,and CAT in serum(all P<0.05)after controlling the effects of smoking and drinking.The higher the activities of GST,Mn-SOD and CAT,the lower the risk of ONHL.The activity of CuZn-SOD was not associated with ONHL(P>0.05).Conclusion The activities of serum antioxidant enzymes such as GST,Mn-SOD and CAT are related to ONHL.Those antioxidant enzymes can be used as effective biomarkers for ONHL workers.
