牙周牙髓综合干预对牙周脓肿患者炎性因子的影响

The Effect of Periodontal and Endodontic Comprehensive Intervention on Inflammatory Factors in Patients With Periodontal Abscess

目的 探讨牙周牙髓综合干预在牙周脓肿患者中的临床效果及对炎性因子的影响。方法 选择2020年7月—2021年5月牙周脓肿患者86例作为对象,随机数字表法分为两组,各43例。对照组采用盐酸米诺环素软膏治疗,观察组联合牙周牙髓综合干预治疗,治疗2周后评估患者效果,比较两组有效率、牙周功能、炎性因子水平。结果 观察组治疗2周后有效率为95.35%,高于对照组72.09%,差异有统计学意义(P <0.05);两组治疗2周后牙周功能得到明显改善;观察组治疗2周后PD、PI、SBI、MD水平低于对照组,差异有统计学意义(P <0.05);两组治疗2周后炎性因子低于干预前,差异有统计学意义(P <0.05);观察组治疗2周后IL-6、IL-8和TNF-α水平低于对照组,差异有统计学意义(P <0.05);治疗前两组患者疼痛程度比较差异无统计学意义(P> 0.05);治疗2周后观察组患者的咬合性疼痛及自发性疼痛程度均明显低于对照组,差异有统计学意义(P <0.05)。结论 牙周牙髓综合干预用于牙周脓肿患者中能提高患者有效率,改善牙周功能,抑制机体炎性反应,缓解患者疼痛程度,值得推广应用。

Objective To explore the clinical effect of comprehensive intervention of periodontal pulp in patients with periodontal abscess and its influence on inflammatory factors.Methods A total of 86 patients with perilunar abscess from July 2020 to May 2021 were selected and divided into two groups by random number table method,with 43cases in each group.The control group was treated with minocycline hydrochloride ointment,while the observation group was treated with periodontal pulp comprehensive intervention.After 2 weeks of treatment,the effect of patients was evaluated,and the effective rate,periodontal function and inflammatory factors were compared between the two groups.Results After 2 weeks of treatment,the effective rate of observation group was 95.35%,which was higher than 72.09% of control group,and the difference was statistically significant(P 0.05).After 2weeks of treatment,the degree of occlusal pain and spontaneous pain in the observation group were significantly lower than those in the control group,and the differences were statistically significant(P <0.05).Conclusion Periodontal pulp comprehensive intervention can achieve high effective rate in patients with periodontal abscess,which is helpful to improve patients’ periodontal function and reduce the level of inflammatory factors,relieve the patient’s pain,and is worthy of popularization and application.
