摘要
目的浅析在早期糖尿病肾病(diabetic nephropathy,DN)患者诊断中,尿微量清蛋白(urine microalbuminuria,UmAlb)+尿转铁蛋白(urine tranferrin,UTRF)+血清胱抑素C(cystatin C,Cys-C)检测价值。方法选择2020年1月-2021年1月丹江口市第一医院收治的2型糖尿病患者30例、早期DN患者60例、同期健康体检者30例,将健康体检者纳入甲组,单纯糖尿病患者纳入乙组,早期DN疾病患者纳入丙组,所有入组者均检测UmAlb、Cys-C、UTRF水平,对检测结果进行组间对比。结果通过表中数据可见:较之甲组,乙组、丙组患者UmAlb、Cys-C、UTRF水平均较高,组间差异有统计学意义(P<0.05);乙组患者各指标水平高于甲组,组间差异有统计学意义(P<0.05)。丙组患者UmAlb、Cys-C、UTRF阳性检出率最高。UmAlb、Cys-C、UTRF联合检测准确度、敏感度、特异度高于单一指标检测。结论应用UmAlb、Cys-C、UTRF联合检测可对早期DN疾病进行检测,明确患者病情,文章的研究结果为早期糖尿病肾病后期诊断标准制定提供了借鉴内容。
Objective To analyze the detection value of urine microalbuminuria(UmAlb)+urine tranferrin(UTRF)+cystatin C(Cys-C)in the diagnosis of early diabetic nephropathy(DN).Methods From January 2020 to January 2021,a total of 30 patients with type 2 diabetes mellitus,60 patients with early DN and 30 healthy subjects admitted to the Danjiangkou First Hospital were selected.The healthy subjects were included in group A,the patients with diabetes alone were included in group B,and the patients with early DN were included in group C.The levels of UmAlb,Cys-C and UTRF were detected in all participants,and the results were compared between groups.Results According to the data in the table,compared with group A,the levels of UmAlb,Cys-C and UTRF in group B and group C were higher,and the differences between groups were statistically significant(P<0.05).The level of each index in group B were higher than that in group A,and the differences between groups were statistically significant(P<0.05).The positive rates of UmAlb,Cys-C and UTRF in group C were the highest.The accuracy,sensitivity and specificity of UmAlb,Cys-C and UTRF combined detection were higher than those of single index detection.Conclusion The combined detection of UmAlb,Cys-C and UTRF can be used to detect early DN disease and clarify the condition of patients.The research results of this paper provide reference for the formulation of late diagnostic criteria of early diabetic nephropathy.
