住院患者医院感染预警及决策支持系统的研发与应用

Development and Application of Hospital Infection Early Warning and Decision Support System for Inpatients

目的 设计研发住院患者医院感染预警及决策支持系统,并评价其在感染控制和护士满意度方面的效果。方法 调取分析2019年1月—2020年12月甘肃中医药大学第三附属医院4 423例住院患者病历资料,根据是否应用医院感染预警及决策支持系统将纳入研究患者分为应用前(2019年1—12月)2 165例和应用后(2020年1—12月)2 258例住院患者。组建研发小组,设计住院患者医院感染预警及决策支持系统,评价应用前后医院感染发生情况,并评价护士对该系统的满意度。结果 应用前发生医院感染113例,占比5.22%,应用后发生医院感染36例,占比1.59%,比较差异有统计学意义(P <0.05),应用前后感染种类占比比较差异无统计学意义(P> 0.05);有效回收的316份调查问卷中,护士对该系统的总体满意度为(90.32±8.20)分,服务质量维度得分为(17.26±3.10)分,净收益维度得分为(19.07±2.68)分,用户满意维度得分为(21.64±2.41)分,信息质量维度得分为(18.59±2.34)分,系统质量维度为(16.71±2.35)分。表明护士对该系统满意度较高。结论 住院患者医院感染预警及决策支持系统的应用整合了评估、预警、决策,提高了护理工作的准确性及效率,医院感染发生情况也有显著降低,值得临床推广。

Objective To design and develop a nosocomial infection early warning and decision support system for inpatients,and to evaluate its effect in infection control and nurse satisfaction.Methods The medical records of 4 423 inpatients in The Third Affiliated Hospital of Gansu University of Traditional Chinese Medicine from January 2019 to December 2020 were retrieved and analyzed.According to whether the hospital infection early warning and decision support system was applied,the patients included in the study were divided into pre-application(January to December 2019)2 165 cases and 2 258 hospitalized patients after application(January to December 2020).Set up a research and development team to design a hospital infection early warning and decision support system for inpatients,evaluated the occurrence of hospital infection before and after application,and evaluated nurses’ satisfaction with the system.Results There were 113 cases of nosocomial infection before application,accounting for 5.22%,and 36 cases of nosocomial infection after application,accounting for 1.59%.The difference was statistically significant(P 0.05).Among the 316 questionnaires that were effectively recovered,the overall satisfaction of the nurses with the system was(90.32±8.20) points.Among them,the dimension of service quality was(17.26±3.10) points,the dimension of net income was(19.07±2.68) points,the dimension of user satisfaction was(21.64±2.41) points,the dimension of information quality was(18.59±2.34) points,and the dimension of system quality was(16.71±2.35) points.It showed that nurses were highly satisfied with the system.Conclusion The application of hospital infection early warning and decision support system for inpatients integrates evaluation,early warning and decision-making,improves the accuracy and efciency of nursing work,and significantly reduces the incidence of nosocomial infection,which is worthy of clinical promotion.
