1例利妥昔单抗治疗抗NMDAR脑炎患儿的药学监护

Pharmaceutical Care For A Child with NMDAR Antibody Encephalitis Treated with Rituximab

目的分析临床药师参与利妥昔单抗治疗抗N-甲基-D-天冬氨酸受体(NMDAR)脑炎患儿的药学监护,为临床药师提供实践参考。方法对1例一线免疫治疗效果不佳的抗NMDAR脑炎患儿的药物治疗实施药学监护,结合患儿病情及药物治疗确定药学监护的要点,对利妥昔单抗的选用、剂量、给药方法、药物相互作用、不良反应等方面提出药学建议,同时对患儿及其家属进行用药指导和出院宣教,并指导其进行定期随访。结果配合临床,实施药学监护,患儿病情好转出院,随访结果显示患儿情况良好。结论临床药师通过对患儿开展全程个体化药学监护,关注患儿使用利妥昔单抗治疗的有效性和安全性,充分体现临床药师的价值。

Objective To analyze the pharmaceutical care of rituximab in treating a child with NMDAR antibody encephalitis by clinical pharmacists and to provide a practical reference for others.Methods To carry out pharmaceutical care for a child with NMDAR antibody encephalitis who failed to improve after first-line immunotherapy,clinical pharmacists combined the illness with drug treatment to determine the main points of the pharmaceutical care and gave pharmaceutical suggestions on the selection of rituximab,dosage,administration,drug interaction,adverse reactions.Meanwhile,clinical pharmacists provided directions on using the medicine and discharge education with regular follow-up guidance to the patient and the patient's families.Results Cooperating with a clinician,clinical pharmacists carried out the pharmaceutical care,and the child got better and was discharged.Follow-up results showed that the patient was in good condition.Conclusion The pharmacist gave individualized pharmaceutical care in the effectiveness and safety of rituximab,demonstrating the value of clinical pharmacists.
