针刺百会八阵穴结合语言康复训练治疗缺血性脑卒中后运动性失语症临床观察

Clinical Observation of Acupuncture at Baihui Bazhen Point Combined with Language Rehabilitation Training in the Treatment of Motor Aphasia after Ischemic Stroke

目的观察针刺百会八阵穴结合语言康复训练治疗缺血性脑卒中后运动性失语症的临床疗效。方法选取缺血性脑卒中后运动性失语症患者84例,随机分为观察组42例、对照组42例,两组均接受舒尔氏(Schuell)刺激法和旋律语调疗法,观察组接受百会八阵穴针刺治疗,对照组接受头针治疗。观察两组治疗前后汉语失语症检查表(ABC)评分、美国国立卫生研究院卒中量表(NIHSS)评分,并统计临床疗效。结果治疗后,两组ABC各项评分均高于治疗前,差异有统计学意义(P<0.05),且观察组明显高于对照组(P<0.05);两组NIHSS评分均低于治疗前,差异有统计学意义(P<0.05),且观察组明显低于对照组(P<0.05);观察组临床疗效总有效率为92.86%,明显高于对照组80.95%(P<0.05)。结论在语言康复训练治疗的基础上给予百会八阵穴阴中隐阳刺法可以明显改善缺血性脑卒中后运动性失语症患者的语言功能(谈话、理解、复述、命名、阅读、书写、结构与空间、运用与计算)及神经功能缺损,临床疗效显著。

Objective To observe the clinical effect of acupuncture at Baihui Bazhen point combined with language rehabilitation training on motor aphasia after ischemic stroke. Methods 84 cases of motor aphasia after ischemic stroke were randomly divided into observation group(42 cases)and control group(42 cases). Both groups received Schuell stimulation and melody intonation therapy. The observation group received acupuncture at Baihui Bazhen point and the control group received scalp acupuncture. The scores of Chinese Aphasia Battery(ABC)and National Institutes of Health Stroke Scale(NIHSS)were observed before and after treatment. Results After treatment,the scores of ABC in the two groups were significantly higher than those before treatment(P<0.05),and the observation group was significantly higher than that in the control group(P<0.05). The NIHSS scores of the two groups were significantly lower than those before treatment(P<0.05),and the observation group was significantly lower than that of the control group(P<0.05). The total effective rate of clinical efficacy in the observation group was 92.86%,which was significantly higher than 80.95% in the control group(P<0.05). Conclusion On the basis of language rehabilitation training,Baihui Bazhen point Yin hidden Yang acupuncture can significantly improve the language function(conversation,understanding,retelling,naming,reading,writing,structure and space,application and calculation)and neurological deficit of patients with motor aphasia after ischemic stroke.The clinical effect is remarkable.
