摘要
目的探讨双胎早产儿之间血清25-羟维生素D[25-(OH)D]水平的影响因素及其与临床结局的关系。方法选取2019年1—12月嘉兴市妇幼保健院新生儿科收治的双胎早产儿74例,根据双胎儿两者血清25-(OH)D水平高低将其分为高维生素D组和低维生素D组,基于配对对照比较两组的早产儿基线资料、并发症及呼吸支持治疗的差异。结果双胎早产儿血清25-(OH)D水平为12.37(17.37~24.54)ng/ml,维生素D缺乏率为59.5%。高维生素D组和低维生素D组的出生顺序、体质量、性别、胎膜早破、脐带绕颈情况、临床并发症发生率之间比较,差异均无统计学意义(均P>0.05),低维生素D组的持续正压气道通气和机械通气时间均高于高维生素D组,差异有统计学意义(P<0.05)。结论双胎早产儿本身维生素D水平低,缺乏率高,在双胎早产儿中,两者维生素D水平的高低与出生顺序、体质量、性别、胎膜早破及脐带绕颈情况无关,与临床并发症的发生率也无关,但双胎低维生素D水平者需要更长的呼吸机辅助通气时间。
Objective To investigate the influencing factors of serum 25-hydroxyvitamin D[25-(OH)D]levels in twin preterm infants and their relationship with clinical outcomes.Methods Seventy-four twin preterm infants admitted to the neonatal unit of Jiaxing Ma⁃ternal and Child Health Hospital from January to December 2019 were selected and divided into high Vit D and low Vit D groups according to the serum 25-(OH)D levels.Based on paired controls,comparison of differences in baseline information,complications and respiratory support treatment of preterm infants between the two groups were taken.Results Serum 25-(OH)D levels in twin preterm infants were 12.37(17.37~24.54)ng/ml,with a Vit D deficiency rate of 59.5%.There was no statistically significant differences between the birth order,born weight,sex,premature rupture of membranes,umbilical cord encirclement,as well as the incidence of clinical complications between the two groups(P>0.05).But the duration of continuous positive airway pressure ventilation and mechanical ventilation in the low Vit D group was longer than that in the high Vit D group,with statistically significant differences(P<0.05).Conclusion The twin pre⁃term infants themselves have low Vit D level and high deficiency rate.In twin preterm infants,the level of Vit D was not related to birth or⁃der,born weight,sex,premature rupture of membranes and umbilical cord encirclement,nor to the incidence of clinical complica⁃tions.But those with low Vit D level in twin preterm infants required longer ventilator-assisted ventilation time.
