呼吸训练联合冲击波治疗产后腰背肌筋膜炎疗效分析

Curative Effect Analysis of Respiratory Training Combined with Shock Wave Therapy in the Treatment of Postpartum Low Back Myofascitis

目的探究呼吸训练联合冲击波治疗对产后背肌筋膜炎的疗效。方法将70例产后腰背肌筋膜炎患者随机分为冲击波组(A组)、呼吸组(B组)、联合组(C组),A组、C组各23例,B组24例。A组予放散式冲击波治疗,B组予呼吸训练,C组予联合治疗。经过4、8、12 w治疗后,比较3组患者疼痛、腰椎功能障碍指数、脊柱活动度、生存质量。结果3组组内治疗各时间点与治疗前相比,各指标评分均存在显著性差异(P<0.05)。3组间治疗前比较,无显著性差异(P>0.05);4 w时脊柱活动度、躯体健康总测量评分组间存在显著性差异(P<0.05);8 w时,疼痛、腰椎功能障碍指数、心理健康总测量评分组间存在显著性差异(P<0.05)。其中C组与A、B组比较存在显著性差异(P<0.05),A、B两组比较无显著性差异(P>0.05)。结论呼吸训练联合冲击波治疗产后腰背肌筋膜炎疗效显著,值得临床推广及进一步研究。

Objective To explore the clinical efficacy of respiratory training combined with shock wave therapy on postpartum chronic low back myofascitis.Methods 70 cases of postpartum low back myofascitis were randomly divided into shock wave group(group A),respiratory group(group B)and combined group(group C),with 23 cases in group A and C,and 24 cases in group B.Group A was given diffuse shock wave therapy,group B was given breathing training,and group C was given combined therapy.After 4,8 and 12 weeks of treatment,pain,lumbar dysfunction index,spinal range of motion and quality of life of the three groups were compared.Results There were significant differences in the scores of each index at each time point of treatment in the three groups compared with before treatment(P<0.05).There was no significant difference among the three groups compared with before treatment(P>0.05);at 4 weeks,there were significant differences in spinal mobility and physical component summary among groups(P<0.05);at 8 weeks,there were significant differences among groups in pain,lumbar dysfunction index and total mental health measurement(P<0.05).There was significant difference between group C and group A and B(P<0.05),and there was no significant difference between group A and group B(P>0.05).Conclusion Respiratory training combined with shock wave therapy has a significant effect on postpartum low back myofascitis,which is worthy of clinical promotion and further study.
