摘要
目的探讨^(18)氟-氟代脱氧葡萄糖/正电子发射计算机断层显像(fluorine-^(18)fluorodeoxyglucose/positron emission tomography-computed tomography,^(18)F-FDG PET-CT)在非霍奇金淋巴瘤中的临床应用价值。方法随机选取2010年1月至2017年1月经病理证实的在锦州医科大学附属第一医院初诊非霍奇金淋巴瘤患者63例,其中包括弥漫大B细胞淋巴瘤、滤泡型淋巴瘤和NK/T细胞淋巴瘤;分别在诊断早期和化疗后不同阶段进行PET/CT检测,分析显像结果与疗效和预后的关系及比较不同组间的差异。结果通过对比患者化疗前后的病灶SUVmax与Dmax值发现,化疗后较前均有显著降低,差异有统计学意义(P<0.05);而化疗后的不同组别间比较,即对不同临床分期和淋巴瘤亚型的比较结果显示,在治疗早期和中期应用PET-CT指导弥漫大B细胞淋巴瘤预后的意义更大;而对于惰性淋巴瘤预后判断和指导治疗的价值不大。结论在不同类型非霍奇金淋巴瘤诊治中正确应用^(18)F-FDG PET/CT显像方法具有重要意义。
Objective To explore the clinical application value of^(18)F-fluorodeoxyglucose(^(18)F-FDG)and positron emission tomography-computed tomography(PET-CT)in the treatment of non-Hodgkin's lymphoma.Methods A total of 63 patients with non-Hodgkin lymphoma newly diagnosed in the First Affiliated Hospital of Jinzhou Medical University from January 2010 to January 2017 were randomly selected and pathologically confirmed,including diffuse large B-cell lymphoma,follicular lymphoma and NK/T cell lymphoma;PET/CT detection was performed at the early stage of diagnosis and at different stages after chemotherapy respectively,to analyze the relationship between imaging results and efficacy and prognosis,and to compare the differences among different groups.Results By comparing the SUVmax and Dmax values of lesions before and after chemotherapy,it was found that there was a significant decrease after chemotherapy,with statistical significance(P<0.05).After chemotherapy,comparison of different clinical stages and lymphoma subtypes was made between different groups,which has showed that the application of PET-CT in the early and middle treatment of diffuse large B-cell lymphoma has greater prognostic significance.However,it is of little value to judge the prognosis and guide the treatment of indolent lymphoma.Conclusion The correct application of^(18)F-FDG PET/CT imaging in the diagnosis and treatment of different types of non-Hodgkin's lymphoma is of great significance.
