健脾止泻汤联合推拿治疗小儿腹泻临床观察

Clinical Observation on Jianpi Zhixie Decoction Combined with Massage in the Treatment of Diarrhea in Children

目的分析健脾止泻汤与推拿在小儿腹泻中的治疗效果及对患儿免疫功能的作用。方法回顾性分析江西省临川区妇幼保健计划生育服务中心2019年1月—2020年12月期间收治的88例腹泻患儿的临床资料,根据患儿采用的治疗方案将其分为A组和B组,每组44例;A组行常规西医治疗,B组行健脾止泻汤结合推拿治疗;对比2组患儿的临床疗效、中医证候积分及血清免疫学指标,包括免疫球蛋白A(IgA)、免疫球蛋白G(IgG)、淋巴细胞CD4^(+)与CD8^(+)的比值(CD4^(+)/CD8^(+))。结果治疗后,B组治疗总有效率为97.73%(43/44),高于A组的86.36%(38/44),差异有统计学意义(P<0.05);B组患儿的中医证候积分均较A组低,差异有统计学意义(P<0.05);B组患儿的CD4^(+)/CD8^(+)、IgA、IgG均较A组高,差异有统计学意义(P<0.05)。结论将健脾止泻汤与中医推拿应用于小儿腹泻的临床治疗中,有助于促进患儿免疫系统的恢复,显著改善发热、腹痛等各项临床症状,有效提高治疗效果。

Objective To analyze the therapeutic effect of Jianpi Zhixie decoction and massage in diarrhea in children and the effect on the immune function of children.Methods The clinical data of 88 children with diarrhea admitted to Linchuan District Maternal and Child Health and Family Planning Service Center from January 2019 to December 2020 were retrospectively analyzed.According to the treatment plan,the children were divided into group A and group B,with 44 cases in each group.Group A received conventional western medicine treatment,and group B received Jianpi Zhixie decoction combined with massage treatment.Clinical efficacy,traditional Chinese medicine(TCM)syndrome score and serum immunological indexes[immunoglobulin A(IgA),immunoglobulin G(IgG),the ratio of lymphocyte CD4^(+)to CD8^(+)(CD4^(+)/CD8^(+))]were compared between the two groups.Results After treatment,the total effective rate of group B was 97.73%(43/44),which was higher than that of group A(86.36%,38/44),and the difference was statistically significant(P<0.05).The TCM syndrome score in group B was lower than that in group A,and the difference was statistically significant(P<0.05).The levels of CD4^(+)/CD8^(+),IgA and IgG in group B were higher than those in group A,and the differences were statistically significant(P<0.05).Conclusion The application of Jianpi Zhixie decoction and traditional Chinese massage in the clinical treatment of infantile diarrhea can promote the recovery of the immune system,significantly improve the clinical symptoms of fever,abdominal pain and so on,and effectively improve the treatment effect.
