摘要
系统分析原发性失眠症(primary insomnia,PI)脑结构和功能的影像学改变,从全脑、脑区、脑区间功能连接和脑网络四个层次分别说明患者的中枢神经系统改变。总结文献发现,PI的发生、发展与海马、杏仁核、扣带回、岛叶、额叶和丘脑等多个脑区的组织结构、功能、代谢变化相关,情绪、认知相关的脑区变化和脑网络功能紊乱与失眠症状和严重程度有关。基于影像学方法和技术研究PI中枢神经系统改变,对理解PI病理生理机制、早期诊断和治疗具有重要的意义。
The imaging changes of the brain structure and function of primary insomnia(PI)were systematically analyzed.The central nervous system changes of the patients were explained from four levels:the whole brain,brain region,brain interval functional connectivity and brain network.The occurrence and development of PI is related to organizational structure,function,and metabolic changes in multiple brain regions,including hippocampus,amygdala,cingulate gyrus,insula,frontal lobe and thalamus.Mood/cognition-related changes in brain regions and brain network dysfunction are related to insomnia symptoms and severity.Studying central nervous system alterations in PI based on imaging methods and techniques has important implications for understanding the pathophysiology of PI,to help the early diagnosis and clinical treatment.
