超声根管预备对慢性牙髓炎根管治疗患儿牙周指数及疼痛的影响

Effects of ultrasonic root canal preparations on the periodontal related indexes and pain in children with chronic pulpitis undergoing a root canal therapy

目的探讨超声根管预备对慢性牙髓炎根管治疗患儿牙周相关指数及疼痛的影响。方法选择76例慢性牙髓炎患儿为研究对象,随机分为研究组与对照组,各38例。研究组给予超声根管预备,对照组给予机用镍钛锉根管预备。比较2组根管预备术前后疼痛评分、根管填充效果、治疗前后牙周相关指数、治疗期间并发症发生率。结果术后3 d 2组咬合疼痛、自发性疼痛VAS评分均升高(P<0.05);研究组术后3 d咬合疼痛、自发性疼痛VAS评分均低于对照组(P<0.05)。研究组根管适充率高于对照组(P<0.05),根管欠充率低于对照组(P<0.05);2组根管超充率比较,差异无统计学意义(P>0.05)。2组菌斑指数(PLI)、牙龈指数(GI)、探诊出血指数(BI)及牙周袋探诊深度(PD)组间、时间、交互比较差异显著(P<0.05);与治疗前比较,2组治疗后即刻和治疗后3个月PLI、GI、BI及PD均降低(P<0.05);治疗后3个月,研究组PLI、GI、BI及PD均低于对照组(P<0.05)。研究组治疗期间并发症发生率低于对照组(P<0.05)。结论慢性牙髓炎根管治疗患儿采用超声根管预备可有效缓解术后疼痛、优化根管填充效果,改善牙周指数,降低并发症发生率。

Objective To investigate the effects of ultrasonic root canal preparations on the periodontal related indexes and pain in children with chronic pulpitis undergoing a root canal therapy.Methods A total of 76 children with chronic pulpitis admitted to our hospital from May 2020 to April 2021 were selected as the research subjects,and they were divided into a study group and a control group,with 38 cases in each group.The study group was given the treatment of ultrasonic root canal preparations,while the control group was given the treatment of mechanical nickel-titanium file root canal preparations.The pain scores before and after root canal preparations,root canal filling effect,periodontal related indexes before and after treatment,and complication rate during treatment were compared between the two groups.Results The VAS scores of occlusal pain and spontaneous pain were increased in both groups 3 days after operation(P<0.05),and the VAS scores in the study group were lower than those in the control group 3 days after operation(P<0.05).The root canal filling rate in the study group was higher than that in the control group(P<0.05),while the root canal underfilling rate was lower than that of the control group(P<0.05).There were no significant differences between the two groups in the root canal overfilling rate(P>0.05).There were statistically significant differences in the plaque index(PLI),gingival index(GI),probing bleeding index(BI)and periodontal pocket probing depth(PD)between the two groups in terms of time and interaction(P<0.05).Compared with before treatment,the PLI,GI,BI and PD in the two groups were decreased immediately after treatment and 3 months after treatment(P<0.05).Three months after treatment,the PLI,GI,BI and PD in the study group were lower than those in the control group(P<0.05).The incidence of complications during treatment was lower in the study group than that in the control group(P<0.05).Conclusion Ultrasonic root canal preparations can effectively relieve postoperative pains,optimize the root canal filling effect,improve the periodontal indexes and reduce the incidence of complications in children with chronic pulpitis undergoing a root canal therapy.
