高频心电图联合运动负荷心电图在隐匿性冠心病诊断中的应用

摘要 目的 探讨高频心电图联合运动负荷心电图在隐匿性冠心病(SMI)诊断中的应用效果。方法 前瞻性选取疑似SMI患者70例,行高频心电图、运动负荷心电图检查,同时行冠状动脉造影(CAG)检查,以CAG检查结果为金标准,分析高频心电图、运动负荷心电图及两者联合诊断SMI的准确度、灵敏度、特异度、阴性预测值(NPV)、阳性预测值(PPV)。结果 经CAG检查结果显示,70例疑似SMI患者中,有58例确诊为SMI,占82.86%;高频心电图联合运动负荷心电图诊断SMI的准确度、灵敏度高于单一检查,差异有统计学意义(P<0.05);高频心电图联合运动负荷心电图诊断SMI的特异度、NPV、PPV与高频心电图、运动负荷心电图单一检查相比,差异无统计学意义(P>0.05)。结论 高频心电图联合运动负荷心电图可提高SMI诊断效能,为SMI的早期治疗提供有效依据。 Objective To explore the application effect of high frequency electrocardiogram combined with exercise stress electrocardiogram in the diagnosis of occult coronary heart disease(SMI).Methods A total of 70 patients with suspected SMI were prospectively selected.They were examined by high-frequency ECG,exercise stress ECG,and coronary angiography(CAG).The results of CAG were taken as the gold standard to analyze the accuracy,sensitivity,specificity and safety of high-frequency ECG,exercise stress ECG and their combination in the diagnosis of SMI Negative predictive value(NPV),positive predictive value(PPV).Results CAG examination showed that 58 out of 70 suspected SMI patients were diagnosed as SMI,accounting for 82.86%;The accuracy and sensitivity of high frequency ECG combined with exercise stress ECG in the diagnosis of SMI were higher than that of single examination,and the difference was statistically significant(P<0.05);There was no significant difference in specificity,NPV and PPV between high frequency ECG and exercise stress ECG in the diagnosis of SMI(P>0.05).Conclusion High frequency ECG combined with exercise stress ECG can improve the diagnostic efficiency of SMI and provide effective basis for the early treatment of SMI.
