心脏超声评估心力衰竭患者病情程度研究

Evaluation of the Severity of Heart Failure by Cardiac Ultrasound

目的 探讨心力衰竭(HF)患者采用心脏超声评估病情程度的临床价值。方法 选择80例HF患者作为研究对象,依据纽约心脏病协会(NYHA)心功能分级将其分为Ⅱ级(n=30)、Ⅲ级(n=26)、Ⅳ级(n=24)。所有患者均采用心脏超声测定3组的左心室射血分数(LVEF)、左心室收缩末期内径(LVESD)、左心室舒张末期内径(LVEDD),分析心脏彩超评估HF患者病情程度的临床价值。结果 Ⅱ级HF患者的LVEF水平最高,LVESD、LVEDD水平最低,其次为Ⅲ级,Ⅳ级HF患者的LVEF水平最低,LVESD、LVEDD水平最高,3组间比较差异有统计学意义(P<0.05);绘制受试者工作特征曲线(ROC)结果显示,LVEF、LVESD、LVEDD评估Ⅲ级、Ⅳ级HF患者的曲线下面积(AUC)均>0.70,有一定预测价值。结论 心脏超声可有效评估HF患者的病情程度,为临床治疗方案制定、患者预后评估提供客观参考。

Objective To explore the clinical value of cardiac ultrasound in evaluating the severity of heart failure(HF).Methods A total of 80 HF patients were selected as the research objects,and they were divided into grade Ⅱ(n=30),grade Ⅲ(n=26),and grade Ⅳ(n=24)according to the New York Heart Association(NYHA)cardiac function classification.LVEF,LVESD and LVEDD were measured by cardiac ultrasound in all patients.The clinical value of cardiac color Doppler ultrasound in evaluating the severity of HF patients was analyzed.Results The LVEF level was the highest in grade Ⅱ HF patients,and the LVESD and LVEDD levels were the lowest,followed by grade Ⅲ.The LVEF level was the lowest in grade Ⅳ HF patients,and the LVESD and LVEDD levels were the highest.The difference among the three groups was statistically significant(P<0.05).ROC results showed that the area under the curve(AUC)of LVEF,LVESD,LVEDD assessment of grade Ⅲ and Ⅳ HF patients were all>0.70,which has certain predictive value.Conclusion Cardiac ultrasound can effectively assess the severity of HF patients,and provide an objective reference for clinical treatment plan formulation and prognosis evaluation of patients.
