摘要
目的 了解天津市居民归因于高盐饮食的相关心脑血管疾病负担,为政府推动减盐相关决策提供数据支持。方法 基于全球疾病负担(GBD)框架模型,利用2015年天津市慢性病与营养监测数据及死因登记系统数据,通过人群膳食钠摄入量来估算归因于高盐饮食的心脑血管疾病死亡的人群归因分值(PAF)、归因死亡人数和归因疾病负担情况。采用SPSS 19.0软件进行独立样本t检验,采用SAS 9.4软件计算疾病负担。结果 天津市25岁及以上人群膳食钠摄入量为(5 920.44±3 615.65)mg/d。2015年天津市户籍人口心脑血管病死亡人数共36 167例,伤残调整寿命年(DALY)为603 361.42人年。天津市25岁及以上人群归因于高盐饮食的心脑血管疾病死亡的PAF为18.22%,归因死亡数为6 590例;归因DALY的PAF为17.37%,归因DALY为104 827.44人年。当人均盐摄入量减少达到WHO推荐标准5 g/d时,即以钠摄入量2 000 mg/d作为理论最小分布,归因于高盐饮食的心脑血管疾病死亡的PAF为14.46%,归因死亡数为5 228例;归因DALY的PAF为13.72%,归因DALY为82 809.79人年。结论 天津市居民食盐摄入量较高,归因于高盐饮食的心脑血管疾病负担较重,应积极开展有针对性的减盐行动。
Objective To understand the disease burden of cardiovascular diseases attributable to high salt diet in Tianjin residents,and provide the data support for promoting salt reduction related policy of government. Methods Based on the Global Burden of Disease(GBD) framework model,Tianjin chronic disease and nutrition monitoring data in 2015 and Tianjin Death Monitoring System were used to estimate the population attribution fraction(PAF),the number of deaths and disease burden of cardiovascular diseases attributable to high salt diet by population dietary sodium intake. The t test was used to analyse the data by SPSS 19.0 software. The SAS 9.4 software was used to estimate the disease burden. Results The dietary sodium intake in Tianjin residents (≥25 years old) was(5 920.44±3 615.65) mg/d. In 2015,the death number of cardiovascular diseases in Tianjin residents was 36 167 cases,disability-adjusted life years(DALY) was 603 361.41 person years. The PAF of cardiovascular diseases death attributable to high salt diet in Tianjin residents (≥25 years old) was 18.22%(6 590 deaths). The PAF of cardiovascular diseases DALY attibutable to high salt diet was 17.37%,the attibutable DALY was 104 827.44 person years. When the per capita salt intake reduced to 5 g/d recommended by WHO,the sodium intake of 2 000 mg/d served as the minimum distribution,PAF of cardiovascular death due to high salt diet was 14.46%,attributable death number was 5 228 cases;PAF of attributable DALY was13.72%,attributable DALY was 82 809.79 person years. Conclusion The salt intake of Tianjin residents is higher,and the disease burden of cardiovascular diseases due to high salt diet is serious. Therefore,the targeted salt reduction actions should be actively performed.
