超声骨刀用于老年牙槽骨修整术的疗效及对患者血清IL-1、IL-6及CRP水平的影响 被引量:3

摘要 目的:探究超声骨刀用于老年牙槽骨修整术的疗效及对患者血清白细胞介素-1(IL-1)、IL-6、C反应蛋白(CRP)的影响。方法:选取90例行牙槽骨修整术的老年患者为研究对象,根据手术工具不同分为传统工具组和超声骨刀组,每组各45例。传统工具组以骨凿等传统工具行牙槽骨修整术;超声骨刀组以超声骨刀行牙槽骨修整术。比较两组患者术前术中心率和平均动脉压;手术时间、术中疼痛度及术后疼痛持续时长;术后3 d血清IL-1、IL-6、CRP水平;术后满意。结果:超声骨刀组患者术中心率及平均动脉压低于传统工具组(P<0.05);手术时间、术中疼痛程度、术后疼痛持续时长低于传统工具组(P<0.05);术后3 d血清IL-1、IL-6、CRP水平低于传统工具组(P<0.05);总满意率高于传统工具组(P<0.05)。结论:超声骨刀可改善老年牙槽骨修整术中不适感,提高手术效率,降低术后炎症反应发生率,提升患者满意度。 Objective: To observe the effect of ultrasonic osteotome in alveoloplasty for the elderly and its influence on serum interleukin-1(IL-1),interleukin-6(IL-6) and C-reactive protein(CRP) levels.Methods:90 elderly patients with alveolar bone repair were selected as the research objects.According to the tool used during operation, the patients enrolled were divided into traditional tool group and ultrasonic osteotome group, with 45 patients in each group.Patients in the traditional tool group were treated with alveoloplasty using traditional tools such as osteotome and bone file, while patients in the ultrasonic osteotome group were treated with alveoloplasty using ultrasonic osteotome.Heart rate and mean arterial pressure of the two groups were monitored before and during the operation.Operation time, intraoperative pain, postoperative pain duration, changes in serum IL-1,IL-6 and CRP levels before and after operation, and patient satisfaction were compared between the two groups.Results:The intraoperative heart rate and increase in mean arterial pressure of the ultrasonic osteotome group were lower than those of the traditional tool group(P<0.05).The operation time, intraoperative visual analogue scale(VAS) score, and postoperative pain duration were shorter or lower than those of the traditional tool group(P<0.05).The increases in serum IL-1,IL-6 and CRP levels at 3 days after operation were smaller than those of the traditional tool group(P<0.05).The overall satisfaction rate in the ultrasonic osteotome group was higher than that in the traditional tool group(P<0.05)Conclusion:The application of ultrasonic osteotome in alveoloplasty for the elderly can significantly relieve the patients’ discomfort, improve the efficiency of operation, reduce the risk of postoperative inflammation, and improve patient satisfaction.
