血清降钙素原联合CRP在血流感染中的诊断预测价值

Predictive Diagnostic Value of Serum Procalcitonin Combined with CRP in Bloodstream Infection

目的:探讨血清PCT联合CRP在血流感染中的诊断预测价值。方法:选取2018年1月-2020年12月血培养阳性患者182例为血流感染组,另选取同期怀疑血流感染但血培养阴性的患者182例为对照组。比较各组血清PCT、CRP水平的差异。应用受试者工作特征曲线和曲线下面积评估PCT、CRP作为诊断指标的诊断效能。结果:血流感染组血清PCT、CRP水平均高于对照组且差异均有统计学意义(P<0.0001)。以对照组为标准,PCT、CRP诊断血流感染的AUC分别为0.821、0.779,两者联合诊断的AUC为0.883。结论:血清PCT、CRP可作为血流感染早期诊断的指标,PCT更为敏感,两者联合测定可提高诊断效能。

Objective:To investigate the predictive diagnostic value of serum PCT combined with CRP in bloodstream infection.Methods:A total of 182 patients with bloodstream infection were analyzed.And 182 patients were selected as negaive group according to the results of blood culture.The levels of serum PCT and CRP were analyred.Receiver operating characteristic curves and the area under the curve were used to evaluate the diagnostic eficacy of PCT,CRP as diagnostic indicators.Results:The levels of serum PCT and CRP in blood culture positive group were signifcanly higher than those in negative group(P<0.0001).The AUC of serum PCT and CRP levels in blood culture positive group were 0821 and 0.779 when compared with the negative group.Combined detection of the two indicators showed the larger AUC(0.883).Conclusion:As the carly diagnostic indicato,scrum PCT has higher sensitivity than CRP.Combined detection of serum PCT and CRP shows higher diagnostic eficacy than individual indicators.
