青光眼是眼科常见疾病。通过案例分析梳理青光眼疾病和手术操作的相应编码。按照前房角形态、病因以及发病年龄,将青光眼分类到H40的各个亚目,先天性青光眼分类到Q15.0,产伤引起的青光眼分类到P15.3;按照不同的术式对青光眼手术分类,小梁切除术分类到12.64,青光眼引流阀植入术分类到12.67,睫状体冷凝术分类到12.72,虹膜激光切开贯通术分类到12.11。编码难点在于要根据分类轴心将疾病分到具体亚目,不但要求编码员掌握编码专业知识,熟悉各种手术术式及步骤,还要学习相关的医学知识,通过详细阅读病历,理解青光眼的疾病性质和相应手术的部位及术式,才能做到准确编码。

Glaucoma is a common ophthalmic disease.In this study,the clinical classification and coding of glaucoma diseases and surgical procedures were systematically reviewed.Glaucoma was accurately classified into all suborders of H40 according to anterior chamber Angle morphology,etiology and onset age,except congenital glaucoma which was classified to Q15.0 and glaucoma caused by birth injury which was classified to P15.3.The correct coding and case analysis were given according to different surgical categories,such as trabeculectomy(12.64),glaucoma drainage valve implantation(12.67),ciliary body condensation(12.72)and iris laser incision(12.11).Glaucoma coding difficulty lies in the axis of disease according to the classification given specific suborder,familiar with all kinds of surgery procedures and steps,in addition to request coders coding professional knowledge,and learning the relevant medical knowledge and careful reading records,accurately understand the nature of the disease of glaucoma and the corresponding operation sites and operation,can achieve accurate coding.
