Hospitalization Expenses Analysis for Rehabilitation Stroke Patients

目的探索脑卒中康复治疗患者住院费用影响因素,为控制医疗费用快速增长和减轻患者经济负担提供依据。方法收集2018年1月1日至2021年8月31日期间重庆市某三级康复专科医院病案信息系统中脑卒中康复治疗患者的722份住院病案首页信息,采用单因素分析、相关性分析及多因素分析筛选住院费用的影响因素。结果脑卒中康复治疗患者住院天数四分位数为31(15,61)天。住院总费用四分位数为28674.19(10587.14,51951.83)元,其中康复治疗费占比最高,超过住院总费用的30%,为9022.19(2851.70,17899.64)元。住院总费用主要受住院天数和医疗付款方式影响,且住院天数与住院总费用呈强正相关。结论脑卒中康复治疗患者的住院费用高,经济负担重,主要受康复治疗费支配。缩短住院天数和控制康复治疗费是减轻患者经济负担的主要手段。

Objectives To explore the influencing factors of hospitalization expenses of patients in the rehabilitation stage of stroke,so as to provide a basis for controlling the rapid growth of medical expenses and reducing the economic burden of patients.Methods 722 front page information of the medical records of inpatients in the rehabilitation stage of stroke in a tertiary specialized hospital in Chongqing from January 1st,2018 to August 31st,2021 was collected,and univariate analysis,correlation analysis and multivariate analysis were adopted to explore the influencing factors of hospitalization expenses.Results The quartile of length of hospital stay was 31(15,61)days.The quartile of hospitalization expenses was 28674.19(10587.14,51951.83)RMB,among them,rehabilitation expenses accounted for the highest proportion,more than 30%of the total hospitalization expenses,9022.19(2851.70,17899.64)RMB.Hospitalization expenses was mainly affected by the length of hospital stay(β=0.824,P<0.05)and the payment method(β=0.233,P<0.05),in addition,there was a strong positive correlation between the length of hospital stay and the hospitalization expenses(r=0.928,P<0.05).Conclusions The hospitalization expense of patients in the rehabilitation stage of stroke was high and the economic burden was heavy,and it was mainly dominated by the expenses of rehabilitation and treatment.Shortening the length of hospital stay,shunting patients to subordinate hospital and conducting PDCA management were the main measures to control the medical expenses.
