电化学发光法检测降钙素原和白细胞介素-6对新生儿细菌感染的诊断价值分析 被引量:6

摘要 目的:研究电化学发光法对新生儿细菌感染(NBI)性疾病检测指标中降钙素原(PCT)、白细胞介素-6(IL-6)的鉴别诊断价值。方法:选取医院收治的105例疑似NBI新生儿纳入观察组,将同期体检的100名正常新生儿纳入健康对照组,两组均采用电化学发光仪检测PCT、IL-6,对比两组不同时间点、不同细菌感染类型的PCT、IL-6水平及诊断效能;分析观察组采用抗生素阶梯疗法进行干预后的PCT、IL-6水平。结果:观察组新生儿出生后4 d行电化学发光检测,其PCT和IL-6水平明显高于健康对照组,差异有统计学意义(t=56.952,t=85.917;P<0.05)。观察组新生儿经抗生素阶梯疗法干预后,PCT和IL-6水平与健康对照组比较差异有统计学意义(t=19.470,t=30.498;P<0.05)。观察组新生儿抗生素干预治疗后PCT和IL-6水平均出现明显下降趋势,与出生后4 d比较,差异有统计学意义(t=49.735,t=68.846;P<0.05)。以细菌培养结果为“金标准”,电化学发光法检测PCT、IL-6的灵敏度分别为91.09%和83.17%,特异度均为50.00%。结论:电化学发光法检测PCT和IL-6应用于新生儿细菌感染中临床效果显著,具有较好诊断效能,可以有效鉴别新生儿细菌感染类型。 Objective:To explore and analyze the diagnostic values of the detection of procalcitonin(PCT)and interleukin-6(IL-6)by using electrochemiluminescence method on neonatal bacterial infection(NBI).Methods:105 children with suspected NBI admitted to hospital were selected as the observation group,and 100 neonates who underwent physical examination at the same period were selected as the health control group.Both two groups adopted electrochemiluminescence to detect PCT and IL-6,and the PCT and IL-6 levels and diagnostic efficiencies of different time point and different type of bacterial infections between two groups were compared.And then,the PCT and IL-6 levels of observation group were analyzed after it adopted gradient therapy with antibiotic to implement intervention.Results:The results of electrochemiluminescence indicated that the levels of PCT and IL-6 in observation group were significantly higher than those in control group(t=56.952,t=85.917,P<0.05)at the 4th day after birth,respectively.After the intervention of gradient therapy with antibiotic,the differences of PCT and IL-6 levels between observation group and control group were significant(t=19.470,t=30.498,P<0.05).The PCT and IL-6 levels of observation group appeared obvious downtrend after neonates underwent intervention treatment with antibiotic,and the differences of these between the 4th day after birth and post intervention were significant(t=49.735,t=68.846,P<0.05),respectively.The result of bacterial culture was used as the"gold standard",and the sensitivities of electrochemiluminescence were respectively 91.09%and 83.17%in detecting PCT and IL-6,and the specificity of that was 50.00%.Conclusion:The clinical effect of electrochemiluminescence is significant in using it to detect PCT and IL-6 of neonates with NBI,which has favorable diagnostic efficacy.It can effectively distinguish the type of bacterial infection of neonates.
