运用品管圈构建医院内窥镜同质化管理模式研究

Research on the construction of hospital endoscope homogenization management mode using quality control circle

目的:运用品管圈(QCC)构建医院内窥镜同质化管理模式,以内窥镜同质化管理的统一标准规范内窥镜地使用,保障患者安全。方法:成立QCC团队,按照主题选定、计划书拟定、课题明确化、目标设定、方案拟定、最适策追究、对策实施、效果确认、标准化和检讨与改进10大步骤进行内窥镜同质化管理的改善,构建医院内窥镜同质化管理模式,建立软式内窥镜同质化规范化培训体系和软式内窥镜精细化管理工作链标准操作规程(SOP)集与“日常质量控制与预防性维护”双螺旋体系,升级改造软式内窥镜保护设备,搭建软式内窥镜全程实时追溯智能化管理多终端平台,实现内窥镜同质化管理数据互通。结果:软式内窥镜大修比例由医院内窥镜同质化管理模式应用前的40.1%降低至应用后的9.6%,内窥镜信息化溯源覆盖率由16.7%提升至100%,软式内窥镜清洗消毒合格率由63.7%提升至97.2%,操作规范率由56.3%提升至93.7%。结论:医院内窥镜同质化管理模式应用于软式内窥镜管理,能够提高设备使用规范率和清洗消毒合格率,降低故障率控制设备维修成本。

Objective:To build a homogenized management mode for hospital endoscopes using the quality control circle(QCC),and to manage and regulate the use of endoscopes based on"uniform standards"to ensure the safety of patients.Methods:The QCC team was set up and improvement activities were performed according to the following 10 steps of theme selection,plan formulation,task clarification,goal setting,plan formulation,investigation of optimal policies,implementation of countermeasures,effect confirmation,standardization,review and improvement.The homogeneity management mode of endoscope in hospital was constructed.A homogenized and standardized training system for flexible endoscopes,a set of standard operating procedures(SOP)for the refined management of flexible endoscopes,and a double-helix system of"routine quality control and preventive maintenance"were established.The flexible endoscope protection equipment was upgraded.A multi-terminal platform for real-time traceability and intelligent management of flexible endoscopes throughout the entire process was built to achieve data interconnection on endoscope homogenization management.Results:The overhaul ratio of flexible endoscopes decreased from 40.1%before the application of the hospital endoscope homogenization management model to 9.6%after the application,the traceability coverage rate of endoscope informatization increased from 16.7%to 100%,the qualified rate of cleaning and disinfection of flexible endoscope increased from 63.7%to 97.2%,and the rate of standardized operation increased from 56.3%to 93.7%.Conclusion:The application of hospital endoscope homogenization management mode to the management of flexible endoscopes can improve the rate of equipment use standardization and the qualified rate of cleaning and disinfection,reduce the failure rate and control equipment maintenance costs.
