摘要
目的:基于安全风险因素分析构建安全盲点挖掘策略,探讨其在医院急危重症患者急救设备管理中的应用价值。方法:选取医院重症医学科临床在用的167台急救设备,按照管理模式的不同将其分别纳入对照组和观察组,对照组采用风险应对管理模式;观察组采用盲点应对管理模式,在分析急救设备26项风险因素的基础上,构建设备故障树和安全概率预测模型,从安全意识、安全制度及管理队伍等层面开展针对性管理。对比两组设备的安全盲点和安全风险比例以及涉及设备使用管理的医务人员业务能力水平。结果:观察组治疗类、诊断类和辅助类急救设备的安全盲点占比分别为3.42%、4.55%和1.09%,均低于对照组,差异有统计学意义(χ^(2)=21.727,χ^(2)=23.878,χ^(2)=69.783;P<0.05);观察组设备第1~4季度的安全风险占比分别为2.84%、2.58%、3.98%和1.41%,均低于对照组,差异有统计学意义(χ^(2)=9.319,χ^(2)=14.931,χ^(2)=6.979,χ^(2)=13.765;P<0.05);观察组急救设备使用管理的相关医生、护士和工程师的考核优秀占比分别为66.67%、77.36%和100.00%,均高于对照组,差异有统计学意义(χ^(2)=6.171,χ^(2)=20.221,χ^(2)=9.000;P<0.05)。结论:安全盲点挖掘策略能够从多个层面分析医院急救设备的安全风险和管理漏洞,有效降低急救设备安全风险发生率,提高设备管理队伍整体水平。
Objective:To construct safety blind spot mining strategy based on the analysis of safety risk factor,and discuss its application value in the management of equipment for rescue of critically ill patients in hospital(referred to as acute and critical equipment).Methods:A total of 167 sets of acute and critical care equipment in clinical use in intensive care unit of the hospital use were selected and divided into control group and observation group according to different management period.The control group adopted the risk response management model.The observation team adopted the blind spot response management model,based on the analysis of 26 risk factors,the equipment fault tree and safety probability prediction model were constructed to carry out targeted management from the aspects of safety awareness,safety system and management team.The safety blind spot and safety risk ratio,as well as the professional capabilities of medical staff involved were compared between the two groups of equipment.Results:The proportion of safety blind spots in the treatment equipment,diagnostic equipment and auxiliary equipment in the observation group were 3.42%,4.55%and 1.09%,respectively,which were lower than those in the control group,the difference was statistically significant(χ^(2)=21.727,χ^(2)=23.878,χ^(2)=69.783,P<0.05).The proportion of safety risk of the equipment in the observation group in the first to fourth quarter was is 2.84%,2.58%,3.98%and 1.41%respectively,which was lower than that in the control group,the difference was statistically significant(χ^(2)=9.319,χ^(2)=14.931,χ^(2)=6.979,χ^(2)=13.765,P<0.05).The excellent percentages of assessment of doctors,nurses and engineers engaged in management of equipment use in the observation group were 66.67%,77.36%and 100.00%respectively,which were higher than those in the control group,the difference was statistically significant(χ^(2)=6.171,χ^(2)=20.221,χ^(2)=9.000,P<0.05).Conclusion:The safety blind spot mining strategy can analyze the safety risks and management loopholes of emergency and critical medical equipment in hospitals from multiple aspects,which can effectively reduce the incidence of safety risks,and improve the overall level of equipment management team.
