小肠Dieulafoy病引起消化道出血外科治疗1例并文献复习

Surgical treatment of gastrointestinal bleeding caused by Dieulafoy disease of the small intestine: a case report and literature review

Dieulafoy病是由于黏膜下动脉畸形引起的一种消化道出血性疾病,好发于胃小弯沿胃的近端近贲门处,但全消化道都有发病的报道,其中小肠Dieulafoy病较为罕见,且诊断困难.其首发症状多为无痛性消化道大出血,甚至引起失血性休克才得以发现.因其出血部位隐匿,诊断困难,内科治疗效果差,常造成治疗延误,危及生命.现经南京中医药大学附属医院消化内镜中心诊断并行外科手术治疗的空肠Dieulafoy病1例报道并文献复习.

Dieulafoy disease is a hemorrhagic disease of the digestive tract caused by submucosal arterial malformation. It occurs in the lesser curvature of the stomach along the proximal part of the stomach near the cardia, but there are reports of the disease in the whole digestive tract. Among them, Dieulafoy disease of the small intestine is more common. Rare and difficult to diagnose. The first symptoms are mostly painless gastrointestinal bleeding, and even cause hemorrhagic shock to be discovered. Because the bleeding site is hidden, he diagnosis is difficult, and the medical treatment effect is poor, which often results in delayed treatment and easily endangers the patient's life. This article reports a case of Dieulafoy's disease of the jejunum diagnosed by endoscopy in our hospital and treated by surgery.
