磁共振相位对比电影成像在脑积水诊断中的临床应用 被引量:1

摘要 目的评价磁共振相位对比电影成像(PC-MRI)技术在脑积水诊断中的临床应用价值。方法使用GE3.0 T磁共振扫描仪,采用PC-MRI程序检查,对我院收治的20例交通性脑积水及25例梗阻性脑积水患者及48名同期在我院体检的志愿者的中脑导水管水平脑脊液流速、流量进行检测,比较2组脑脊液相关力学数据基本情况。结果 交通性脑积水中脑导水管水平脑脊液在不同阶段所引起的反应存在着一定的差异,其中在收缩期与舒张期分别表现为向下、向上峰速,而且均大幅增加了其流量;梗阻性脑积水中脑导水管水平脑脊液在不同阶段所引起的反应也存在一定的差异,其中在收缩期与舒张期分别表现为向下、向上峰速,而且在流量方面都出现大幅降低的现象。相较于健康人进行统计有明显差异(P<0.05),健康人(对照组)脑脊液流动曲线呈U形、倒U形,正弦波形,曲线光滑,而脑积水组脑脊液流动曲线呈锯齿状,无规则,或呈直线。结论 磁共振相位对比电影成像能无创检测脑脊液流速、流量和方向,可鉴别脑积水类型,为临床治疗提供方向。 Objective To evaluate the clinical application value of magnetic resonance phase-contrast film imaging(PC-MRI) technology in the diagnosis of hydrocephalus. Methods The cerebral aqueduct level cerebrospinal fluid flow rates and flow in 20 cases with traffic hydrocephalus, 25 cases with obstructive hydrocephalus and 48volunteers during the same period in our hospital were detected using GE3.0 T magnetic resonance scanner and PCMRI program,then compare the two groups′ CSF related mechanical data. Results The reaction of horizontal cerebrospinal fluid in different stages, including downward and upward peak speed during systolic and diastolic periods, which greatly increased the flow;the reaction of horizontal cerebrospinal fluid in different stages, including downward and upward peak speed in flow. Compared with normal people, there are obvious differences( P<0.05), the cerebrospinal F flow curve of normal people(control group) is U-shaped, inverted U shape, sinusoidal waveform, the curve is smooth, while the cerebrospinal F flow curve of hydrocephalus group is serrated, irregular, or straight. Conclusion MR phase contrast film imaging can noninvasive detect CSF velocity, flow and direction, identify hydrocephalus type and provide direction for clinical treatment.
