IBD合并EB病毒感染患儿外周血 NF-κB与病情进展的关系 被引量:1

Relationship between the expression of peripheral blood NF-κB and disease progression in children with IBD and EB virus infection

摘要 目的 探讨炎症性肠病(IBD)合并人类疱疹病毒4型(EB)病毒感染患儿外周血核转录因子κB(NF-κB)与病情进展的关系。方法 选取2018年1月-2021年1月中国医科大学附属盛京医院收治的IBD合并EB病毒感染患儿88例,根据病情进展不同将其分为IBD活动组(n=46)和IBD缓解组(n=42),观察患儿疾病活动指标,另外分别抽取两组患儿5 ml静脉血,采取酶联免疫吸附法检测两组肿瘤坏死因子(TNF-α)水平,并利用逆转录聚合酶链反应(RT-PCR)检测血清中NF-κB水平。结果 IBD活动组临床疾病活动指数(CAI)评分、血沉、C-反应蛋白(CRP)及病理组织学评分分别为(12.84±3.27)分、(50.74±16.84)mm/h、(33.17±13.52)mg/L及(9.47±2.76)分均高于IBD缓解组(P<0.001);IBD活动组血清中NF-κB和TNF-α分别为(2.15±0.53)和(135.17±28.56)pg/ml均高于IBD缓解组(P<0.001);血清中TNF-α和NF-κB表达与IBD活动度均呈正相关(P<0.05)。结论 血清TNF-α和NF-κB与IBD合并EB病毒感染患儿的发生发展均呈正相关,临床可将其作为评估IBD合并EB病毒感染病情进展的重要参考指标。 OBJECTIVE To investigate the relationship between peripheral blood nuclear factor κB(NF-κB) and disease progression in children with inflammatory bowel disease(IBD) and Epstein-Barr(EB) virus infection. METHODS A total of 88 children with IBD and EB virus infection were admitted to Shengjing Hospital Affiliated to China Medical University from Jan. 2018 to Jan. 2021. They were divided into active group(n=46) and remission group(n=42) according to the progression of IBD. Disease activity indicators of the children were observed, and venous blood samples were collected to determine the level of tumor necrosis factor(TNF-α) by enzyme-linked immunosorbent assay. Serum NF-κB level was detected by reverse transcription-polymerase chain reaction(RT-PCR). RESULTS The clinical disease activity index(CAI) score, erythrocyte sedimentation rate, C-reactive protein(CRP) level and histopathological score in the active group were(12.84±3.27) points,(50.74±16.84) mm/h,(33.17±13.52) mg/L and(9.47±2.76) points, higher than those in the remission group(P<0.001). Serum NF-κB and TNF-α levels in the active group were(2.15±0.53) and(135.17±28.56) pg/ml, higher than those in the remission group(P<0.001). Serum TNF-α and NF-κB expression was positively correlated with the activity of IBD(P<0.05). CONCLUSION Serum TNF-α and NF-κB are positively correlated with the occurrence and development of children with IBD and EB virus infection. Clinically, these indicators can be used as reference indicators for evaluating the progression of IBD complicated with EB virus infection.
