低温等离子射频消融腺样体切除术联合扁桃体部分切除术治疗儿童OSAS的效果及对心理行为、生活质量的影响

Effect of low temperature plasma radiofrequency ablation adenoidectomy combined with partial tonsillectomy on children with OSAS and its influences on psychological behavior and quality of life

目的探讨低温等离子射频消融腺样体切除术联合扁桃体部分切除术治疗儿童阻塞性睡眠呼吸暂停综合征(OSAS)的效果。方法选取2018年1月至2020年9月我院收治的90例OSAS患儿作为研究对象,根据治疗方案不同将其分为对照组和研究组,各45例。对照组给予扁桃体腺样体切除术,研究组给予低温等离子射频消融腺样体切除术联合扁桃体部分切除术。比较两组的治疗效果。结果术后6个月,研究组的睡眠呼吸暂停低通气指数(AHI)、呼吸紊乱指数(RDI)低于对照组,最低血氧饱和度(LSaO_(2))高于对照组(P<0.05)。术后6个月,研究组的心理行为各项评分均低于对照组(P<0.05)。研究组的治疗总有效率高于对照组(P<0.05)。术后6个月,研究组儿童阻塞性睡眠呼吸暂停疾病特异性生活质量调查表18(OSA-18)各项评分均低于对照组(P<0.05)。结论低温等离子射频消融腺样体切除术联合扁桃体部分切除术治疗儿童OSAS能有效改善睡眠呼吸相关指标,减轻心理行为问题,提高患儿术后的生活质量。

Objective To explore the effect of low temperature plasma radiofrequency ablation adenoidectomy combined with partial tonsillectomy on children with obstructive sleep apnea syndrome(OSAS).Methods A total of 90 children with OSAS admitted in our hospital from January 2018 to September 2020 were selected as the research objects,the children were divided into control group and study group according to different treatment schemes,with 45 cases in each group.The control group was given adenotonsiuectomy,and the study group was given low temperature plasma radiofrequency ablation adenoidectomy combined with partial tonsillectomy.The therapeutic effects of the two groups were compared.Results At 6 months after operation,the apnea-hypopnea index(AHI)and respiratory disorder index(RDI)in the study group were lower than those in the control group,and the lowest oxygen saturation(LSaO_(2))was higher than that in the control group(P<0.05).At 6 months after operation,the psychological and behavioral all items scores in the study group were lower than those in the control group(P<0.05).The total effective rate of treatment in the study group was higher than that in the control group(P<0.05).At 6 months after operation,the Disease Specific Quality of Life for Children with Obstructive Sleep Apnea 18 Items Survey(OSA-18)all items scores in the study group were lower than those in the control group(P<0.05).Conclusion Low temperature plasma radiofrequency ablation adenoidectomy combined with partial tonsillectomy for children with OSAS can effectively improve sleep respiration related indicators,alleviate psychological and behavioral problems,improve the quality of life of children after operation.
