乳腺癌术后切口感染病原菌及其影响因素

Pathogenic bacteria and its influencing factors of postoperative incision infection in breast cancer patients

目的分析乳腺癌患者术后切口感染病原菌及其影响因素。方法回顾性分析2019年6月-2021年6月南阳市中心医院356例乳腺癌手术患者的临床资料,依据患者是否发生术后切口感染分为感染组(n=29)和对照组(n=327),对感染组患者病原菌分离鉴定,归纳乳腺癌妇女术后切口感染的影响因素。结果29例乳腺癌术后切口感染患者共培养分离病原菌38株,其中革兰阴性菌22株占57.89%,革兰阳性菌15株占39.47%,真菌1株占2.63%;Logistic多因素分析显示,伴有应激性高血糖、白蛋白、未预防性应用抗菌药物是乳腺癌妇女术后切口感染的影响因素(P<0.05)。结论应激性高血糖、白蛋白和抗菌药物应用情况为乳腺癌妇女术后切口感染的影响因素,临床中应对高危患者加强调控。

OBJECTIVE To analyze the pathogenic bacteria and its influencing factors of postoperative incision infection in breast cancer patients.METHODS The clinical data of 356 patients undergoing breast cancer surgery in Nanyang Central Hospital from Jun.2019 to Jun.2021 were retrospectively analyzed.The patients were divided into infection group(n=29)and control group(n=327)according to whether postoperative incision infection occurred.Pathogenic bacteria in the infection group were isolated and identified,and the influencing factors of postoperative incision infection in women with breast cancer were summarized.RESULTS Thirty-eight strains of pathogenic bacteria were cultured and isolated from 29 patients with incision infection after breast cancer surgery,of which 22 strains of gram-negative bacteria accounted for 57.89%,15 strains of gram-positive bacteria accounted for 39.47%,and 1 strain of fungus accounted for 2.63%.Logistic multivariate analysis showed that the combined stress hyperglycemia,albumin,and non-prophylactic use of antibiotics were the influencing factors of postoperative incision infection in women with breast cancer(P<0.05).CONCLUSION Stress hyperglycemia,albumin and the use of antibiotics were the influencing factors of postoperative incision infection in women with breast cancer,and high-risk patients should be strengthened in clinical practice.
