卵巢癌患者化疗并发感染影响因素及外周血SIL-2R与GDF15和CXCL5对其诊断价值 被引量:2

Influencing factors of chemotherapy complicated infection in ovarian cancer patients and their diagnostic value of peripheral blood SIL-2R,GDF15 and CXCL5

摘要 目的探讨卵巢癌患者化疗并发感染的影响因素及其外周血可溶性白细胞-2受体(SIL-2R)、生长分化因子15(GDF15)、趋化因子配体5(CXCL5)对其诊断价值。方法选取2017年5月-2020年4月承德市中心医院400例卵巢癌化疗患者作为研究对象,统计感染发生率,分析感染与未感染患者临床资料、外周血SIL-2R、GDF15、CXCL5水平,归纳卵巢癌化疗并发感染的影响因素,采用受试者工作特征(ROC)曲线评价外周血SIL-2R、GDF15、CXCL5水平对卵巢癌化疗并发感染的诊断价值,随访6个月,比较联合预测阳性与阴性患者预后情况。结果400例卵巢癌化疗患者共82例并发感染,感染率为20.50%;年龄、化疗周期、抗菌药物预防及化疗后外周血SIL-2R、GDF15、CXCL5是卵巢癌化疗患者并发感染影响因素(P<0.05);化疗后外周血SIL-2R、GDF15、CXCL5水平联合诊断卵巢癌化疗患者并发感染的曲线下最大面积(AUC)最大,具有良好诊断效能;随访6个月,联合诊断阳性患者死亡率较阴性患者高(P<0.05)。结论卵巢癌患者化疗并发感染与年龄、化疗周期、抗菌药物预防及化疗后外周血SIL-2R、GDF15、CXCL5密切相关;SIL-2R、GDF15、CXCL5可用于其临床诊断及预后预测,采取针对性治疗方案,改善患者预后。 OBJECTIVE To investigate the influencing factors of chemotherapy-complicated infection in patients with ovarian cancer and their diagnostic value of peripheral blood soluble leukocyte-2 receptor(SIL-2 R),growth differentiation factor 15(GDF15)and chemokine ligand 5(CXCL5).METHODS A total of 400 ovarian cancer patients undergoing chemotherapy in Chengde Central Hospital Hospital from May 2017 to Apr.2020 were selected as the research subjects,the incidence of infection was counted,the clinical data,peripheral blood SIL-2 R,GDF15,and CXCL5 levels of infected and uninfected patients were analyzed,and the influencing factors of chemotherapy-complicated infection in ovarian cancer patients were summarized,the receiver operating characteristic(ROC)curve was used to evaluate the diagnostic value of peripheral blood SIL-2 R,GDF15 and CXCL5 levels for chemotherapy-complicated infection in ovarian cancer patients.After 6 months of follow-up,the prognosis of patients with positive and negative combined predictions was compared.RESULTS A total of 82 cases of 400 ovarian cancer patients undergoing chemotherapy were complicated with infection,and the infection rate was 20.50%.Age,chemotherapy cycle,antibiotic prophylaxis,and peripheral blood SIL-2 R,GDF15,and CXCL5 after chemotherapy were the influencing factors of complicated infection in ovarian cancer patients undergoing chemotherapy(P<0.05);After chemotherapy,peripheral blood SIL-2 R,GDF15,and CXCL5 levels had the largest area under the curve(AUC)in the combined diagnosis of ovarian cancer chemotherapy patients complicated with infection,which had a good diagnostic performance.After 6 months of follow-up,the mortality rate of patients with positive combined diagnosis was higher than that of patients with negative combined diagnosis.(P<0.05).CONCLUSION Infections complicated by chemotherapy in patients with ovarian cancer were closely related to age,chemotherapy cycle,antimicrobial prophylaxis,and peripheral blood SIL-2 R,GDF15,and CXCL5 after chemotherapy;SIL-2 R,GDF15,and CXCL5 could be used for its clinical diagnosis and prognosis prediction,and targeted treatment plans could be adopted to improve the prognosis of patients.
