经皮电刺激治疗帕金森病患者功能性便秘的临床研究

Clinical Study of Transcutaneous Electrical Stimulation on Functional Constipation in Patients with Parkinson Disease

目的:探讨经皮电刺激(transcutaneous electrical stimulation,TES)对帕金森病(Parkinson disease,PD)患者功能性便秘的治疗作用。方法:选取2021年7-12月镇江市第四人民医院神经内科的PD患者76例,采用随机数字表法将患者分为对照组和试验组,每组38例。对照组给予常规治疗,试验组在对照组的基础上给予TES治疗,治疗时间为2周。比较两组每周完全自主排便(complete spontaneous bowel movements,CSBMs)次数;比较两组胃动素和胃泌素水平、胃窦运动指数及患者便秘状况评估问卷(patient assessment of constipation quality of life questionnaire,PAC-QOL)评分。结果:治疗后第2周,试验组CSBMs次数多于对照组;治疗2周后,试验组胃动素水平高于对照组(P<0.05),两组胃泌素水平比较,差异无统计学意义(P>0.05);治疗2周后,试验组胃窦运动指数高于对照组(P<0.05);治疗2周后,试验组PAC-QOL评分低于对照组(P<0.05)。结论:TES治疗显著促进PD患者的胃肠动力,对PD患者功能性便秘的治疗有显著的效果。

Objective:To investigate the effect of transcutaneous electrical stimulation(TES)on functional constipation in patients with Parkinson disease(PD).Method:A total of 76 patients with PD admitted to the Department of Neurology of the Fourth People’s Hospital of Zhenjiang from July to December 2021 were selected and divided into control group and experimental group by random number table method,with 38 patients in each group.The control group was given conventional treatment,and the experimental group was given TES treatment on the basis of the control group.The treatment time was 2 weeks.The number of weekly complete spontaneous bowel movements(CSBMs)was compared between the two groups.The levels of motilin and gastrin,antral motility index and the patient assessment of constipation quality of life questionnaire(PAC-QOL)scores were compared between the two groups.Result:At the second week after treatment,the number of CSBMs in the experimental group was more than that in the control group.After 2 weeks of treatment,the motilin level in the experimental group was higher than that in the control group(P<0.05),there was no significant difference in the gastrin level between the two groups(P>0.05).After 2 weeks of treatment,the antral motility index of the experimental group was higher than that of the control group(P<0.05).After 2 weeks of treatment,the PAC-QOL score of the experimental group was lower than that of the control group(P<0.05).Conclusion:TES treatment significantly promotes gastrointestinal motility in patients with PD,and has a significant effect on the treatment of functional constipation in patients with PD.
