TB-IGRA在不同年龄段肺结核患者中的应用研究

Application of TB-IGRA in Patients with Pulmonary Tuberculosis at Different Ages

目的:研究结核感染T细胞γ干扰素释放试验(TB-IGRA)在不同年龄段肺结核患者诊断中的应用。方法:选取2021年1月-2022年3月麻城市人民医院接收的128例疑似为肺结核患者作为此次研究对象,根据患者的年龄不同划分为青年组(18~44岁,共25例)、中年组(45~59岁,共50例)、老年组(≥60岁,共53例),三组均进行痰涂片抗酸染色试验检测、TB-IGRA检测。对比痰涂片抗酸染色试验、TB-IGRA检测的阳性检出率,同时,以结核菌培养为“金标准”,对比TB-IGRA在不同年龄段肺结核中的诊断效能。结果:TB-IGRA检测的青年组、中年组、老年组阳性率分别为92.00%、96.00%、96.23%,均高于痰涂片抗酸染色试验检测的48.00%、48.00%、58.49%(P<0.05);在TB-IGRA检测下,青年组(95.45%、88.00%、33.33%)、中年组(97.83%、92.00%、25.00%)、老年组(98.00%、94.34%、33.33%)的敏感度、准确度、特异度对比差异均无统计学意义(P>0.05)。结论:TB-IGRA在肺结核中具有较高的诊断价值,无论是在青年、中年群体还是老年群体,该检测方法的诊断敏感度较高。

Objective:To study the application of T-cell interferonγrelease test(TB-IGRA)in diagnosis of tuberculosis patients at different ages.Method:A total of 128 suspected tuberculosis patients admitted in Macheng People’s Hospital from January 2021 to March 2022 were selected as the subjects of this study.According to the age of the patients,they were divided into youth group(18-44 years old,with 25 cases),middle-aged group(45-59 years old,with 50 cases)and elderly group(≥60 years old,with 53 cases).Acid fast staining test and TB-IGRA test were performed in all three groups.The positive rate of sputum smear acid fast staining test and TB-IGRA detection were compared.At the same time,TB culture was taken as the"gold standard"to compare the diagnostic efficacy of TB-IGRA in different age groups of tuberculosis.Result:The positive rates of TB-IGRA in young group,middle-aged group and elderly group were 92.00%,96.00%and 96.23%,respectively,which were higher than 48.00%,48.00%and 58.49%of sputum smear acid-fast staining test(P<0.05).Under the TB-IGRA assay,there were no significant differences in sensitivity,accuracy and specificity among young group(95.45%,88.00%,33.33%),middle-aged group(97.83%,92.00%,25.00%)and elderly group(98.00%,94.34%,33.33%)(P>0.05).Conclusion:TB-IGRA has high diagnostic value in pulmonary tuberculosis,and its diagnostic sensitivity is high in young,middle-aged and elderly patients.
