反复呼吸道感染患儿血清IgA、总IgE、VitA、25-(OH)D及微量元素的变化研究 被引量:2

摘要 目的:探究与观察反复呼吸道感染患儿血清免疫球蛋白A(IgA)、总免疫球蛋白E(IgE)、维生素A(VitA)、25羟维生素D[25-(OH)D]及微量元素的变化情况。方法:选取2021年10月-2022年7月广州市增城区新塘医院收治的80例反复呼吸道感染患儿为观察组,同期体检的80例健康儿童为对照组。检测与比较两组血清IgA、总IgE、VitA、25-(OH)D及微量元素[锌(Zn)及铁(Fe)]水平,并比较观察组不同性别、分期(发作期及缓解期)、分类(反复上呼吸道感染及反复下呼吸道感染)及严重程度(轻度、中度及重度)患儿的血清IgA、总IgE、VitA、25-(OH)D及微量元素水平。结果:观察组血清IgA、VitA、25-(OH)D及微量元素水平均低于对照组,总IgE水平高于对照组,差异均有统计学意义(P<0.05);观察组不同性别患儿的血清IgA、总IgE、VitA、25-(OH)D及微量元素水平比较,差异均无统计学意义(P>0.05);观察组发作期的血清IgA、VitA、25-(OH)D及微量元素水平均低于缓解期,总IgE水平高于缓解期(P<0.05);观察组反复下呼吸道感染的血清IgA、VitA、25-(OH)D及微量元素水平均低于反复上呼吸道感染,总IgE水平高于反复上呼吸道感染,差异均有统计学意义(P<0.05);重度反复呼吸道感染患儿的血清IgA、VitA、25-(OH)D及微量元素水平均低于中度和轻度患儿,且中度患儿以上指标水平均低于轻度患儿,总IgE水平高于中度和轻度患儿,且中度患儿总IgE水平高于轻度患儿,差异均有统计学意义(P<0.05)。结论:反复呼吸道感染患儿的血清IgA、总IgE、VitA、25-(OH)D及微量元素水平显著异于健康儿童,且不同分期、分类及严重程度患儿的差异较大,因此在本病患儿中的临床检测价值较高。 Objective:To investigate and observe the changes of serum immunoglobulin A(IgA),total immunoglobulin E(IgE),vitamin A(VitA),25-hydroxyvitamin D[25-(OH)D]and microelement of children with recurrent respiratory tract infection.Method:A total of 80 children with recurrent respiratory tract infection who were admitted to Xintang Hospital of Zengcheng District in Guangzhou City from October 2021 to July 2022 were selected as the observation group,and 80 healthy children who were examined at the same time were selected as the control group.The levels of serum IgA,total IgE,VitA,25-(OH)D and microelement[zinc(Zn)and ferritin(Fe)]were detected and compared between the two groups.The levels of serum IgA,total IgE,VitA,25-(OH)D and microelement in the observation group with different gender,stage(attack stage and remission stage),classification(recurrent upper respiratory tract infection and recurrent lower respiratory tract infection)and severity degree(mild,moderate and severe)were compared.Result:The levels of serum IgA,VitA,25-(OH)D and microelement in the observation group were lower than those of control group,and the total IgE level was higher than that of control group,the differences were statistically significant(P<0.05);there were no statistically significant differences in the levels of serum IgA,total IgE,VitA,25-(OH)D and microelement between the gender in the observation group(P>0.05);the levels of serum IgA,VitA,25-(OH)D and microelement in the attack stage of observation group were lower than those in the remission stage,and the total IgE level was higher than that in the remission stage(P<0.05);the levels of serum IgA,total IgE,25-(OH)D and microelement of recurrent lower respiratory tract infection in the observation group were lower than those of recurrent upper respiratory tract infection,and the total IgE level was higher than that of recurrent upper respiratory tract infection,the differences were statistically significant(P<0.05);the levels of serum IgA,VitA,25-(OH)D and microelement in children with severe recurrent respiratory tract infection were lower than those of the moderate and mild cases,and the levels of above indicators of moderate cases were lower than those of the mild cases,the total IgE level was higher than that of the moderate and mild cases,and that of moderate cases was higher than that of the mild case,the differences were statistically significant(P<0.05).Conclusion:The serum IgA,total IgE,VitA,25-(OH)D and microelement of children with recurrent respiratory tract infection are significantly different from those of healthy children,there are significant differences among children with different stages,classifications and severity degree.Therefore,the clinical detection value in children with recurrent respiratory tract infection is high.
