温经养血汤与艾灸疗法应用于子宫内膜异位症痛经患者的前瞻性随机研究

Prospective Randomized Trial of Wenjing Yangxue Decoction and Moxibustion in Dysmenorrhea Patients with Endometriosis

目的:探讨温经养血汤与艾灸疗法应用于子宫内膜异位症痛经患者的效果,以便临床寻找出更有效的治疗方案。方法:选取2019年12月-2021年9月荆门市中医医院收治的子宫内膜异位症痛经患者120例,依据随机数字表法将其分为对照组和试验组,各60例。对照组给予艾灸疗法,试验组在对照组的基础上给予温经养血汤治疗,两组均连续治疗3个月。比较两组临床疗效,治疗前后中医症候积分、血液流变学、血清前列腺素F_(2α)(PGF_(2α))、催产素(OT)、神经生长因子(NGF)水平。结果:治疗后,试验组临床总有效率(93.33%)高于对照组(80.00%);与治疗前比,治疗后两组痛经、小腹肿块、下腹疼痛、形寒肢冷评分和全血高切黏度、全血低切黏度、红细胞沉降率、血清PGF_(2α)、OT、NGF水平均降低,且试验组均低于对照组,差异均有统计学意义(P<0.05)。结论:子宫内膜异位症痛经患者应用温经养血汤与艾灸治疗,可有效缓解其临床症状,降低血清PGF_(2α)、OT、NGF水平,改善血液流变学,且疗效显著。

Objective:To investigate the effect of Wenjing Yangxue Decoction combined with moxibustion therapy in dysmenorrhea patients with endometriosis in order to find out a more effective treatment option clinically.Method:A total of 120 patients with dysmenorrhea due to endometriosis who were admitted to Jingmen Hospital of Traditional Chinese Medicine from December 2019 to September 2021were selected.They were divided into the control group and the experimental group according to the random number table methed,with 60 cases in each group.The control group was given moxibustion therapy,and the experimental group was given Wenjing Yangxue Decoction on the basis of the control group.The two groups received continuous treatment for three months.Two groups were compared in clinical efficacy after treatment,TCM syndrome scores,blood rheology,and serum prostaglandin F_(2α)(PGF_(2α)),oxytocin(OT)and nerve growth factor(NGF)levels before and after treatment.Result:After treatment,the total clinical effective rate of patients in the experimental group(93.33%)was higher than that of the control group(80.00%).Compared with those before treatment,the scores of dysmenorrhea,lower abdomen mass,lower abdominal pain,cold limbs due to cold constitution and high whole blood shear viscosity,low whole blood shear viscosity,erythrocyte sedimentation rate,and serum PGF_(2α),OT and NGF were all decreased in the two groups after treatment,those in the experimental group were lower than those in the control group with statistically significant differences(P<0.05).Conclusion:Treatment with Wenjing Yangxue Decoction combined with moxibustion therapy in dysmenorrhea patients with endometriosis effectively relieves its clinical symptoms and decreases serum PGF_(2α),OT and NGF levels,improve blood rheology,and are highly effective.
