替考拉宁的不同用药方案治疗耐甲氧西林金黄色葡萄球菌肺炎患者的效果差异对比

Comparison of the Effects of Different Regimens of Teicoplanin in the Treatment of Methicillin Resistant Staphylococcus Aureus Pneumonia

目的:探讨替考拉宁的不同用药方案治疗耐甲氧西林金黄色葡萄球菌(MRSA)肺炎的效果。方法:选取2018年12月-2020年4月南京市高淳人民医院收治的MRSA肺炎患者200例,按照不同用药方案分组,其中A组68例,予以小剂量替考拉宁治疗;B组65例,予以大剂量替考拉宁治疗;C组67例,予以大剂量替考拉宁联合美罗培南治疗。比较三组炎症指标、肝肾功能指标、临床疗效、病原菌清除效果、不良反应发生情况。结果:治疗后,三组炎症指标均低于治疗前,B组和C组的炎症指标均低于A组,且C组均低于B组(P<0.05);治疗后,三组肝肾功能指标比较,差异均无统计学意义(P>0.05)。B组和C组的总有效率均高于A组(P<0.05)。B组和C组病原菌清除效果均优于A组(P<0.05)。C组不良反应发生率低于A、B组(P<0.05)。结论:大剂量替考拉宁和大剂量替考拉宁联合美罗培南治疗MRSA肺炎的治疗显现出了优异的临床效果,值得推荐。

Objective:To investigate the effects of different regimens of Teicoplanin in the treatment of methicillin resistant Staphylococcus aureus(MRSA)pneumonia.Method:A total of 200 patients with MRSA pneumonia admitted to Nanjing Gaochun People’s Hospital from December 2018 to April 2020 were selected and grouped according to different medication regimens,and 68 cases in group A were treated with low-dose Teicoplanin,65 cases in group B were treated with high-dose Teicoplanin,67 cases in group C were treated with high-dose Teicoplanin combined with Meropenem.Inflammation indicators,liver and kidney function indicators,clinical efficacy,pathogen removal effect and adverse reaction were compared among the three groups.Result:After treatment,the levels of inflammatory indicators among the three groups were lower than those before treatment,and the inflammatory indicators in group B and group C were lower than those in group A(P<0.05).After treatment,the differences of levels of liver and kidney function indicators among the three groups were not statistically significant(P>0.05).The total effective rates in group B and group C were higher than that in group A(P<0.05).The pathogen removal effects in group B and group C were better than that in group A(P<0.05).The incidence of adverse reactions in group C was significantly better than those in group A and B(P<0.05).Conclusion:High-dose Teicoplanin and high-dose Teicoplanin combined with Meropenem show excellent clinical effect in the treatment of MRSA pneumonia,and are worthy of recommendation.
