慢性心力衰竭患者外周血lncRNA MIAT和miR-214水平变化及与预后关系 被引量:1

Changes of lncRNA MIAT and miR-214 levels in peripheral blood of patients with chronic heart failure and their relationships with prognosis

摘要 目的观察慢性心力衰竭(CHF)患者外周血长链非编码RNA(lncRNA)心肌梗死相关转录物(MIAT)和miR-214表达情况及与预后关系。方法收集2018年1月至2021年3月舞阳县人民医院的50例CHF患者(观察组)和同期体检的30例健康受试者(对照组)的血浆标本。采用实时荧光定量PCR(qRT-PCR)检测两组血浆中lncRNA MIAT和miR-214的相对表达水平,采用彩色多普勒超声检测两组左室舒张末期内径(LVEDD)和左室射血分数(LVEF)。根据随访结果将观察组进一步分为终点事件组和无终点事件组,分析外周血lncRNA MIAT和miR-214水平变化与预后关系。结果观察组BNP、lncRNA MIAT、LVEDD水平明显高于对照组,miR-214、LVEF明显低于对照组,差异均有统计学意义(P<0.05)。终点事件组年龄、脑钠肽(BNP)、lncRNA MIAT水平明显高于无终点事件组,miR-214水平明显低于无终点事件组,差异均有统计学意义(P<0.05)。相关性分析显示,CHF患者的lncRNA MIAT水平与LVEDD、BNP水平呈正相关(P<0.05),与LVEF、miR-214呈负相关(P<0.05);miR-214水平与LVEDD、BNP水平呈负相关(P<0.05),与LVEF呈正相关(P<0.05)。受试者工作特征(ROC)曲线分析显示,lncRNA MIAT+miR-214预测CHF患者预后的曲线下面积(AUC)为0.890,高于BNP、miR-214单独预测的AUC,差异均有统计学意义(P<0.05)。结论lncRNA MIAT可以靶向miR-214调控CHF的发生和发展,对CHF患者预后预测具有一定的价值,可能成为预测CHF患者预后的标志物。 Objective To observe the expression of long non-coding RNA(lncRNA)myocardial infarct-associated transcript(MIAT)and miR-214 in peripheral blood of patients with chronic heart failure(CHF)and their relationships with prognosis.Methods From January 2018 to March 2021,plasma was collected from 50 CHF patients(observation group)and 30 healthy subjects(control group)who underwent physical examination during the same period in Wuyang People′s Hospital.Quantitative real-time PCR(qRT-PCR)was used to detect the relative expression levels of lncRNA MIAT and miR-214 in plasma of the two groups,and echocardiography was used to detect left ventricular end-diastolic diameter(LVEDD)and left ventricular ejection fraction(LVEF)in the two groups.According to the follow-up results,the observation group was further divided into end-point event group and non-end-point event group,and the relationship between the changes of lncRNA MIAT and miR-214 levels in peripheral blood and prognosis was analyzed.Results The levels of brain natriuretic peptide(BNP),lncRNA MIAT and LVEDD in the observation group were significantly higher than those in the control group,while miR-214 and LVEF were significantly lower than those in the control group,and the differences were statistically significant(P<0.05).The age,BNP,lncRNA MIAT and levels of the end point event group were significantly higher than those of the non-end point event group,and miR-214 was significantly lower than that of the non-end point event group,and the differences were statistically significant(P<0.05).Correlation analysis showed that the level of lncRNA MIAT of CHF patients was positively correlated with LVEDD and BNP(P<0.05),and negatively correlated with LVEF and miR-214(P<0.05).The level of miR-214 was negatively correlated with LVEDD and BNP(P<0.05),and positively correlated with LVEF(P<0.05).Receiver operating characteristic(ROC)curve analysis showed that the area under the curve(AUC)of lncRNA MIAT+miR-214 in predicting the prognosis of CHF patients was 0.890,which was higher than that of BNP and miR-214 alone,and the differences were statistically significant(P<0.05).Conclusion LncRNA MIAT could target miR-214 to regulate the occurrence and progression of CHF,which has certain value in predicting the prognosis of CHF patients and may become a marker for predicting the prognosis of CHF patients.
