摘要
目的系统评价中国老年人糖尿病前期(pre-diabetes mellitus,PDM)的患病率。方法检索JBI循证卫生保健中心、Cochrane Library、PubMed、知网、维普等数据库2022年6月以前老年人PDM的相关研究,使用Stata 16.0软件进行Meta分析。结果纳入33篇文献,包含503671例研究对象。Meta分析结果显示,中国老年人PDM总患病率为22.6%[95%CI(17.1%,28.1%)]。亚组分析结果显示,女性患病率略高于男性,老年人患病率随年龄增长而上升,西北、华南、华北、华东地区患病率较其他地区高,使用2010年美国糖尿病协会诊断标准较1999年世界卫生组织诊断标准的患病率更高,空腹血糖结合口服葡萄糖耐量实验比单纯空腹血糖筛查出的患病率更高。结论不同特征的老年人PDM患病率不同,筛查方式和诊断标准对患病率有影响,应重视老年人群PDM的筛查和防治。
Objective To systematically evaluate the prevalence of pre-diabetes mellitus(PDM)among Chinese elderly.Methods JBI evidence-based health care center database,Cochrane Library,PubMed,CNKI,VIP and other databases were searched to collect studies of elderly PDM before June 2022.Stata 16.0 was used for meta-analysis.Results A total of 33 articles covering 503671 subjects were included.According to the findings of the meta-analysis,there were 22.6%[95%CI(17.1%,28.1%)]of senior Chinese people overall who had PDM in that country.The results of the subgroup analysis revealed that women had a somewhat higher prevalence rate than men.The prevalence of the elderly increased with the increase of age,and the prevalence of the elderly in northwest,southern,northern and eastern China was higher than that in other regions.The prevalence of the elderly using the 2010 diagnostic criteria of the American Diabetes Association was higher than that of the 1999 diagnostic criteria of the World Health Organization,and the prevalence of the elderly using the fasting glucose combined with oral glucose tolerance test was higher than that of the fasting glucose alone.Conclusions The elderly with different characteristics have different prevalence rates of PDM,and screening methods and diagnostic criteria have an impact on the prevalence rate.Attention should be paid to the screening and prevention of PDM in the elderly population.
